Help reimagine mental health care for older adults!

A fast-growing digital health company is expanding its behavioral health services and seeking licensed therapists who care deeply about supporting older adults.

Our care model combines human care navigation, therapy, and technology-enabled support to help older adults and caregivers manage emotional health concerns, chronic conditions, cognitive changes, social isolation, and complex healthcare needs.

You’ll work within an established care-support infrastructure, allowing you to focus more of your time on clinical care and less on administrative coordination.

What you’ll do

As a remote therapist, you will:

Provide evidence-based psychotherapy through secure telehealth to adults 60+ and, when clinically appropriate, their caregivers.
Develop and refine individualized treatment plans that take into account medical conditions, caregiving circumstances, cognitive functioning, and social determinants of health.
Collaborate closely with our healthcare advocates and our AI-powered tools to:
Monitor clinical progress
Identify early warning signs of decline or crisis or changes in mood, functioning, safety and engagement between visits
Support adherence to treatment recommendations and broader care plans
Maintain timely and accurate electronic documentation in a streamlined EHR/care management platform.
Share feedback that helps improve clinical workflows and the patient experience.

What you bring

Active, unrestricted license as an LCSW, LICSW, LMHC, LCPC, LPC, or LMFT in at least one U.S. state.
At least one year of independent, post-licensure psychotherapy experience without required supervision including a) Supporting older adults, caregivers, or Medicare populations; b) Treating concerns related to grief, isolation, chronic illness, life transitions, dementia, or mild cognitive impairment.
Experience providing therapy to adults through evidence-based approaches such as CBT, ACT, DBT, IPT, supportive therapy, or related modalities.
Prior telehealth experience or comfort providing care through secure video.
Strong documentation, communication, and collaboration skills.
Interest in working within a growing, fast-moving care model.

Nice-to-haves

Licensure in multiple states, particularly NY, PA, NJ, CA,TX, FL, KY, MN, MA, WA or eligibility to practice through an applicable interstate compact.
Experience in home health, senior living, skilled nursing, FQHC, integrated care, or other settings serving medically complex adults.
