Full Job Description

Job Summary

Join our dedicated home health and hospice team as a Part TIme and Weekend On Call Registered Nurse (RN), where your expertise will provide vital patient care during evenings, nights, weekends, and holidays. As an on-call RN, you will respond promptly to urgent patient needs, coordinate care plans, and collaborate with interdisciplinary teams to uphold our commitment to exceptional patient-centered care. Your proactive approach and clinical skills will make a meaningful difference in the lives of individuals requiring critical support during non-traditional hours.

Responsibilities

Provide immediate assessment and intervention for patients experiencing urgent or emergent health issues during on-call hours.
Coordinate with physicians, family members, and caregivers to develop or adjust care plans based on patient needs and triage assessments.
Administer medications, including IV infusions and injections, following aseptic technique protocols to ensure patient safety.
Conduct comprehensive patient evaluations, including vital signs, physical examinations, and symptom assessments related to hospice or home health care.
Document all patient interactions accurately within electronic health record (EHR) system, ensuring compliance with HIPAA regulations.
Educate patients and families on medication administration, wound care, infection control practices, and disease management strategies.
Collaborate with interdisciplinary teams—including social workers, therapists, and physicians—to ensure seamless continuity of care

Qualifications

Current Idaho RN licensure in good standing.
Experience in home health nursing and hospice care
Strong clinical skills in IV infusion therapy, medication administration (including botulinum toxin treatment when applicable), suturing, and aseptic technique.
Ability to perform comprehensive patient assessments, including vital signs, physiology knowledge, and physical examinations.
Flexibility to work weekends, holidays, and overnight shifts as part of the on-call schedule; must possess basic life support certification.

Pay: $36.00 - $40.00 per hour
