We are seeking an experienced and motivated Occupational Therapist (OT) to join our growing rehabilitation team serving Skilled Nursing Facilities (SNFs) throughout Northern Utah. This is an ideal opportunity for a clinician who enjoys variety, autonomy, and the ability to positively impact patients across multiple facilities.

You'll work with supportive facility teams while helping patients regain independence, improve daily function, and achieve their highest level of performance.

Compensation Package Industry-Leading Pay

$60 per hour based on experience

What You'll Do

Evaluate residents and develop individualized occupational therapy treatment plans.
Provide skilled therapy services to improve activities of daily living (ADLs), functional mobility, upper extremity function, cognition, and safety awareness.
Complete initial evaluations, progress notes, recertifications, and discharge documentation.
Collaborate with interdisciplinary teams including physicians, nursing staff, physical therapists, speech-language pathologists, and facility administrators.
Recommend adaptive equipment and environmental modifications to maximize independence and safety.
Educate patients, families, and caregivers regarding treatment goals and home programs.
Maintain compliance with Medicare, state, and facility documentation standards.

Qualifications Required

Graduate of an accredited Occupational Therapy program.
Current Utah Occupational Therapist license in good standing.
Current CPR/BLS certification.
Valid driver's license, reliable transportation, and current auto insurance.
Ability and willingness to travel between facilities throughout Northern Utah.
Strong communication and documentation skills.

Preferred

Skilled Nursing Facility (SNF) experience.
Experience with Medicare Part A and Part B therapy services.
Experience treating geriatric and post-acute rehabilitation populations.
