We are seeking a dynamic and compassionate Site Coordinator to join our Residential Crisis Unit, dedicated to providing immediate support and stabilization for individuals experiencing mental health crises. Essential Job Functions
- Provides coordination and support for the daily operations of the Residential Crisis Unit and promotes a safe, therapeutic, and structured environment for individuals receiving services.
- Serves as a primary point of contact for intake and referral calls, including calls from individuals, families, hospitals, emergency departments, community providers, behavioral health agencies, and other referral sources.
- Obtains and reviews information necessary to determine appropriateness for admission and coordinates the admission process in accordance with program criteria, policies, and applicable regulatory requirements.
- Coordinates timely admissions, transfers, and discharges in collaboration with the clinical team, referral sources, hospitals, community providers, and other applicable parties.
- Ensures required admission and discharge documentation is completed accurately and within established timeframes.
- Coordinates communication regarding client needs, presenting concerns, safety risks, medications, treatment recommendations, and other pertinent information with appropriate members of the treatment team.
- Monitors the residential census and assists with maintaining accurate admission, discharge, referral, and utilization records.
- Provides clinical coordination and support consistent with the scope of practice and requirements of a Qualified Mental Health Professional–Adult (QMHP-A).
- Monitors individuals for changes in mental health, behavioral, or safety concerns and ensures appropriate crisis prevention, intervention, de-escalation, and referral supports are provided.
- Supports staff in responding to behavioral health crises and maintaining a safe environment for individuals, staff, and visitors.
- Coordinates with behavioral health providers, hospitals, outpatient providers, case managers, families, and community partners throughout the individual's episode of care.
- Maintains professional relationships and linkages with community partners and referral sources to support continuity of care and appropriate transitions from the Residential Crisis Unit.
Documentation & Compliance
- Conducts routine audits of client charts and progress notes to ensure compliance with agency policies, clinical standards, licensing requirements, Medicaid requirements, and applicable DBHDS regulations.
- Reviews progress notes and other clinical documentation for accuracy, completeness, timeliness, appropriate clinical content, required signatures, and consistency with the individual's treatment plan and services provided.
- Identifies documentation deficiencies and communicates required corrections to appropriate staff.
- Monitors completion of required assessments, treatment plans, service notes, discharge summaries, safety plans, crisis plans, and other required clinical documentation.
- Tracks outstanding documentation and follows up with staff to ensure corrections are completed within established timeframes.
- Assists with quality assurance activities, internal audits, external audits, licensing reviews, and corrective action plans as requested.
- Collects, enters, monitors, and reports required administrative, clinical, census, and service-utilization data in accordance with established protocols.
- Ensures documentation and service delivery are consistent with applicable HIPAA, DBHDS, Medicaid, Medicare, and agency requirements.
Program Coordination
- Coordinates daily workflow and communication among clinical, direct-care, nursing, administrative, and supervisory staff.
- Participates in staff meetings, treatment team meetings, case reviews, quality assurance meetings, and other required meetings.
- Assists with identifying operational, staffing, documentation, and service-delivery concerns and communicates concerns to the appropriate supervisor or program leadership.
- Supports implementation of program policies, procedures, clinical standards, and quality improvement initiatives.
- Assists with orientation and training of staff regarding documentation, program procedures, crisis response, and compliance requirements as assigned.
- Promotes person-centered care and supports individuals in developing skills related to coping, communication, emotional regulation, daily living, and successful transition to community-based services.
- Supports coordination of referrals and linkages to medically necessary behavioral health services and community resources upon admission and discharge.
- Maintains accurate and confidential client records and protects client rights and privacy at all times.
- Complies with all agency policies and procedures, HIPAA requirements, DBHDS regulations, Medicaid/Medicare requirements, and applicable federal and state laws.
- Maintains regular attendance and punctuality and follows all policies and procedures regarding scheduled work time and leave.
- Completes required training within prescribed timeframes.
- Demonstrates the agency's valued behaviors of accountability, commitment, compassion, professionalism, and caring.
- Performs other related duties as assigned.
What We Are Looking For
- Qualified Mental Health Professional–Adult (QMHP-A) required.
- Bachelor's degree in human services, behavioral health, psychology, social work, counseling, or a closely related field from an accredited institution.
- Minimum of one year of relevant full-time experience in behavioral health, mental health, crisis services, residential services, or a related field.
- Master's degree in Counseling, Social Work, Psychology, or a closely related field preferred.
- Current, unrestricted Virginia license as an LPC, LCSW, LMFT, or other applicable behavioral health professional preferred.
- Experience working in a residential crisis, crisis stabilization, behavioral health, or similar setting preferred.
- Experience conducting clinical documentation and chart audits strongly preferred.
- Knowledge of behavioral health documentation standards, treatment planning, crisis intervention, and quality assurance processes.
- Knowledge of Virginia's behavioral health system of care and applicable DBHDS requirements.
- Knowledge of Medicaid behavioral health documentation and service requirements preferred.
- Demonstrated ability to assess information, identify documentation/compliance concerns, and follow through with corrective action.
- Strong organizational skills and ability to manage multiple priorities in a fast-paced residential crisis environment.
- Excellent verbal and written communication skills.
- Ability to effectively communicate with individuals served, families, referral sources, community providers, and multidisciplinary staff.
- Proficiency with electronic health records, Microsoft Office Suite, web-based data entry systems, and other computer applications.
- Must possess a valid, unrestricted Virginia driver's license and maintain required automobile insurance if transportation is required for the position.
Pre-Employment Screenings & Required Training
- Satisfactory completion of required criminal background and employment screenings in accordance with agency policy.
- Satisfactory TB screening, as required.
- Current certification in CPR/First Aid or ability to obtain certification within the required timeframe.
- Completion of agency-required crisis intervention/de-escalation training.
- Completion of required agency and regulatory training, including HIPAA/confidentiality, human rights, emergency preparedness, incident reporting, person-centered practices, crisis response, and other training applicable to the position.
Working Environment & Physical Demands
- Work is performed primarily within a residential crisis facility and may include community-based activities and meetings.
- Must be able to communicate effectively with individuals served, families, staff, providers, and community partners.
- Must be able to review and evaluate written documentation for accuracy, completeness, and compliance.
- Must be able to remain calm and professional while working with individuals experiencing behavioral health crises or emotional distress.
- May be required to assist with crisis prevention, intervention, and de-escalation in accordance with agency policy and approved training.
- Must be able to observe and respond appropriately to changes in an individual's behavioral or mental health presentation.
- Ability to sit, stand, walk, bend, and move throughout the facility as required to perform job duties.
- Physical requirements, including lifting or assisting individuals, will be consistent with agency policy and the essential functions of the position.
- May be required to work evenings, weekends, holidays, or participate in an on-call or rotating coverage schedule based on program needs.
- Exposure to individuals experiencing acute behavioral health symptoms and crisis situations should be anticipated.
Position Expectations
The Site Coordinator is expected to serve as a reliable clinical and operational resource for the Residential Crisis Unit. The position requires strong attention to detail, sound judgment, effective communication, and the ability to balance client care, admissions coordination, staff support, documentation compliance, and regulatory requirements while maintaining a safe and therapeutic environment.
Pay: From $65,000.00 per year
