Full Job Description

CC Young, a vibrant senior living community and a non-profit leader in senior living and Health Care has immediate opening for a high-energy Registered Nurse for our Hospice In-Patient Unit.

Available PRN shifts: 6 a.m. - 6 p.m. and 6 p.m. - 6 a.m.

Job duties include, but are not limited to:

Conducts comprehensive assessments of patient’s physical, emotional and spiritual needs.
Develops and implements individualized care plans in collaboration with interdisciplinary teams.
Administers medications and treatments as prescribed and monitors patient responses, adjusting care plans as needed.
Works closely with physicians, social workers, chaplains, and other healthcare professionals to coordinate holistic care and participates in regular team meetings to discuss patient progress and adjusts care plans as needed.
Educate patients and their families about hospice care, symptom management and available support services.
Maintains accurate and up-to-date patient records.
Responds to emergent situations with composure and provides immediate care and support to patients and families during critical times.
Performs other duties as assigned.

Minimum qualifications include:

Must be a Registered Nurse (RN) in good standing in the State of Texas.
Must possess a minimum of 2+ years of recent experience as an RN Charge Nurse in a fast passed hospice/skilled nursing environment.
Must possess up to date IV skills, including IV push medications.
Proven clinical assessment skills with the ability to develop/implement customized care plans and perform procedures as needed.
Strong knowledge of hospice care principles, pain management, and ethical considerations in end-of-life care.
Excellent verbal, written, customer service and interpersonal skills.
Proven computer skills for performing extensive patient documentation and charting.
