Full Job Description

Case Manager provide case management services to participants receiving Community Support (CS) service. Current CS services include access to housing navigation, tenancy and sustainability services, housing deposits, meals/medically tailored meals, and respite services. CMI's also coordinate care with Case Managers II and III/ECM lead care manager to access housing, medical, social, education, and other services. This includes direct assistance in gaining access to CS services, coordination of care, development and oversight of a case plan, and linkage to appropriate services. Case Management is on behalf id the program participant and provides management of the case in collaboration with client.

SUPERVISION RECEIVED AND EXERCISED

The Case Manager I classification receives direction from the Program Manager. Employees designated as Case Manager Level II and III may also provide additional guidance and instruction to the Level I classification, at the direction of the Program Manager.

ESSENTIAL FUNCTION STATEMENTS

Essential responsibilities and duties may include, but are not limited to, the following:

Development of an individualized integrated care plan; including housing plan.
Referral and related activities to help the program participant obtain needed services.
Conduct regular case plan and update as needed for change in need for services.
Documentation of services in case plan.
Conduct a needs assessment of program participant/member to determine eligibility and appropriate services, needs (i.e., ECM/CS services)
Assist with TAR submission for ECM/CS members/clients.

QUALIFICATIONS

Knowledge of:

Principle and practices of leadership, motivation, team building, and conflict resolution.
Principles of business writing, basic presentations, proper grammar, spelling, punctuation, and composition of participant and agency documents.
Office procedures, business mathematics application, computer office equipment and procedures and statistical recordkeeping methods.
Principles and procedures of record keeping, reporting, and filling systems.
Principles, practices and knowledge of Federal and State, and local laws of residential programs.
CalAIM Enhanced Case Management (ECM) and Community Support (CS) services.

Ability to:

Provide services that meet Culturally and Linguistically Appropriate Services (CLAS) Standards, that are respectful of and responsive to each person's culture and communication needs, and consider the client's cultural health benefits, preferred language(s), health literacy levels, and other communication needs.
Consistently exercise discretion and judgement to analyze case plans and decides what actions are necessary.
Maintain an active case load providing interventions as needed.
Collaborate with program team to establish foundation for program participant's case plan.
Work with participant on a day-to-day basis using professional judgement and discretion to guide participant through case plan.
Assist in development, implementation and revision of case/treatment plan and monitor progress towards goals.
Consult and corporate with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining treatment goals.

Provide crisis management and individual and group activities.

Required Skills:

Education/Work Experience:

Any combination of experience and training that would likely provide the required knowledge and abilities in qualifying. A typical way to obtain knowledge and abilities would be:

Minimum Requirements

High School Diploma or GED, and;
1-2 years social service experience or;
Associate degree in social services or related field or;
AOD Counselor Certification or registered intern recognized by DHCS.

Physical Demands:

Essential and marginal functions may require maintaining physical condition necessary for sitting or standing for prolonged periods of time in both indoor office and external program environment; occasional bending and stopping, computer, and operating assigned equipment. Reasonable accommodations for physical requirements will be considered and made on a case-by-case basis. Requests for reasonable accommodation may be made to human resources.

Special Requirements:

Must possess current and valid California driver’s license, including a driving record acceptable to F&H insurance company.
Must pass criminal background screening.
Must meet minimum requirements to access and use program data entry systems.
Completion of a pre-employment drug screening and post-employment drug or alcohol tests upon reasonable suspicion of use.
Must be insurable by F&H insurance carriers.
Be available for call-back to program as needed.
Provide required proof of legal right to work in the United States.
Must provide T.B. clearance from a medical professional.
Must provide DMV print out.
Must be able to support a flexible schedule, including evening, nights, weekends and graveyard shifts.
Shall maintain CPR/First Aid Certification
