Position Overview:
Leads a multidisciplinary team to provide holistic, person-centered care for a diverse health plan population. Acting as the primary contact for members, caregivers, and providers, the RN Care Manager assesses, develops, and monitors individualized care plans to optimize health outcomes across the care continuum.
Key Responsibilities:
- Lead a multidisciplinary team—including social workers, dietitians, pharmacists, and clinical support staff—to deliver care telephonically and digitally.
- Assess members’ health, psychosocial needs, cultural preferences, and support systems.
- Develop and implement individualized care plans in partnership with members and caregivers, addressing gaps and barriers to improve outcomes.
- Coordinate resources such as community services, mental health support, disease-specific programs, and transportation or durable medical equipment.
- Provide education on health literacy, self-management, medications, and nutrition.
- Advocate for members while promoting self-advocacy and engagement in care.
- Monitor and adjust care plans as needed, documenting interactions and progress accurately.
- Facilitate smooth transitions from facilities to home or other care settings, ensuring continuity of care.
- Maintain compliance with professional standards and pursue ongoing development, including Case Management certification (CCM).
Job Qualifications:
- Nursing Diploma or Associate’s degree required; Bachelor’s degree strongly preferred.
- 3+ years of clinical nursing experience in acute, post-acute, or community settings.
- Case management experience in a managed care setting preferred.
- Experience with telephonic and digital patient management preferred.
- Current, unrestricted Michigan RN license required; CCM certification required or to be obtained within 18 months. CCP certification preferred.

- Excellent time management, organizational, and communication skills.
- Comfortable managing multiple systems and working both independently and collaboratively.
- Proficient in Microsoft Office Suite (Excel, Word, Outlook, Teams, OneNote).

Pay: $36.00 - $40.00 per hour
