Three Rivers Hospice is looking for a compassionate and dedicated Registered Nurse (RN) Case Manager to join our growing team in Licking, MO.

Responsibilities

Manage and coordinate hospice care for an assigned caseload of patients.
Perform comprehensive nursing assessments and develop individualized care plans.
Collaborate with physicians, social workers, chaplains, aides, and other members of the interdisciplinary team.
Provide education and emotional support to patients and their families.
Maintain accurate documentation and ensure compliance with hospice regulations.
Advocate for patients while promoting comfort, dignity, and quality of life.

Qualifications

Current Missouri Registered Nurse (RN) license.
Hospice or home health experience preferred but not required.
Excellent assessment, communication, and organizational skills.
Compassionate, dependable, and committed to providing exceptional end-of-life care.
Valid driver's license and reliable transportation.
