Full Job Description

San Diego Family Circle Adult Day Health Center is looking for a long term, inspiring, positive, outgoing, full time Monday - Friday 40 Hours a Week social worker who truly appreciates both working in the San Diego health care community and with a frail elderly population.

Education:

Have a minimum education of a bachelor’s degree in social work from an accredited school with two years of experience providing social work services in one or more of the fields of aging, health or long-term services.

Social Worker Duties include:

Initial Assessments and reassessments of participants;
Case management services;
Daily documentation of services provided;
Development of a plan of care;
And other duties as required by the administrator and/or program director.

Must work well within an interdisciplinary team, enjoy working with the elderly, have good communication skills and writing abilities.

In addition, the position requires a health examination signed by a physician and a TB clearance no more than 12 months prior to employment and a current certificate of CPR and First Aid training.

Must be able to speak, read and write English fluently, bilingual, a plus.

Job Type: Full-time

Pay: $32.00 per hour

Expected hours: 40.0 per week
