Full Job Description

Overview
We are seeking a dynamic and compassionate Clinical Liaison (LVN or RN) to join our growing wound care group. This individual will serve as the clinical and educational bridge between our wound care providers and partner Skilled Nursing Facilities (SNFs), ensuring excellence in wound care delivery, education, and relationship management.

The ideal candidate will be a licensed nurse with a strong understanding of skin and wound care, excellent interpersonal skills, and a proactive approach to problem-solving and collaboration.

Key Responsibilities:

Conduct Skin Sweeps:

Perform routine skin sweeps in assigned SNFs to identify new or worsening wounds, assess current wound care practices, and ensure appropriate documentation and escalation.

Clinical Education & In-Services:

Deliver regular in-service education sessions for SNF clinical staff on wound prevention, treatment protocols, documentation standards, and wound care best practices.

Relationship Management:

Serve as the primary clinical point of contact for SNF leadership and nursing staff. Build strong, trust-based relationships to support high-quality patient outcomes and program success.

Liaison Duties:

Coordinate between SNF staff, wound care providers, and internal operations teams to ensure seamless communication, care delivery, and follow-up.

Documentation & Reporting:

Maintain accurate records of skin sweeps, education sessions, and facility interactions. Provide reports and feedback to internal leadership and SNF partners as needed.

Quality Assurance:

Monitor wound care trends and outcomes in SNFs, identifying opportunities for clinical improvement and ensuring adherence to evidence-based practices.

Qualifications:

Required:
Active LVN or RN license in the state of CA
Minimum of 2 years of clinical nursing experience (preferably in wound care or long-term care)
Strong knowledge of wound types, staging, prevention, and treatment
Ability to educate and influence clinical staff at all levels
Excellent communication, organization, and interpersonal skills
Valid driver’s license and reliable transportation (travel between SNFs required)
Preferred:
Wound care certification (e.g., WCC, CWCN, or equivalent)
Experience in a liaison, educator, or consulting role
Familiarity with EMR systems and clinical documentation practices in SNFs
