Job Description:

Under direct supervision, a Care Lead is an entry-level management position responsible for providing home care services to the patients/clients of Dignified Hope Care Inc. (DHC). A Care Lead must maintain one of the following certification(s) or license(s): CNA, EMT, or MA certification. Care Leads work closely within a supportive upper management team to train staff and oversee the development and coordination of patient care plans. Their responsibilities include ensuring alignment between care teams, fostering a collaborative environment, and ensuring that patient care plans are tailored to meet individual needs. Through effective leadership, they help guide the team in delivering high-quality care, driving continuous improvement, and optimizing patient outcomes.

EMPLOYMENT STANDARDS

· Must possess and maintain a current and valid Certified Nursing Assistant (CNA), Emergency Medical Technician-Basic (EMT-B), or Medical Assistant (MA) certificate.

·Must have experience within the scope of their license.

· Must possess and maintain a current valid Basic Life Support for the Healthcare Providers (CPR/BLS) certification provided by the American Heart Association (AHA) or an equivalent certifying organization.

· Must possess and maintain a current and valid Class “C” State of California Driver License with an acceptable driving history.

· Must be available to work full-time.

Pay: $20.00 - $23.00 per hour
