We are seeking an experienced Registered Nurse Case Manager to provide clinical coordination, support field clinicians and ensure that patients receive timely, appropriate and compliant care.
Position Summary
The RN Case Manager coordinates and oversees care for an assigned home health patient caseload from admission through discharge. This position works closely with nurses, therapists, home health aides, physicians, patients and authorized representatives to ensure the Plan of Care is followed and changes in patient condition are addressed promptly.
The ideal candidate will have experience caring for high-acuity or medically complex patients and will be prepared to work independently after completing required agency-specific orientation and onboarding.
This hybrid position generally includes three to four days per week in the Clovis office and one to two days completing field visits, assessments or supervisory visits. The schedule may change depending on patient and operational needs.
Essential Responsibilities
Patient Care Coordination
- Coordinate care for an assigned patient caseload from admission through recertification, transfer or discharge.
- Develop, review and help implement individualized Plans of Care in collaboration with physicians and the interdisciplinary care team.
- Ensure ordered visit frequencies, treatments and services are scheduled and completed appropriately.
- Monitor patient progress, clinical outcomes, changes in condition, hospitalizations, emergency room visits and other significant events.
- Identify changes in condition and determine when clinical intervention or escalation is necessary.
- Communicate changes in patient condition to the physician and appropriate members of the care team.
- Follow up on missed visits, declined services, scheduling barriers and other issues that could affect patient safety or continuity of care.
- Provide patients and authorized representatives with education and assistance in navigating home health services.
Clinical Oversight
- Review clinical documentation for completeness, accuracy, timeliness and consistency with the Plan of Care.
- Monitor physician orders, medication profiles, durable medical equipment, supplies and other patient care needs.
- Review OASIS assessments and other clinical documentation as assigned.
- Conduct patient assessments, supervisory visits and field visits when needed.
- Provide clinical guidance to RNs, LVNs, therapists, home health aides and other members of the care team.
- Monitor patients with complex medication regimens, wound-care needs, infusion services, multiple diagnoses or frequent hospitalizations.
- Escalate urgent clinical concerns to the appropriate clinical leader or physician.
- Assist with patient complaints and support the timely resolution of care-related concerns.
Authorizations and Recertifications
- Monitor authorization periods, visit utilization and payer requirements.
- Coordinate recertifications and reauthorizations to prevent interruptions in medically necessary services.
- Ensure supporting documentation is completed and submitted within required timeframes.
- Collaborate with intake, authorization, scheduling and billing staff to resolve barriers affecting patient care.
- Monitor service utilization to ensure services remain medically necessary and consistent with the Plan of Care.
Interdisciplinary Collaboration
- Participate in interdisciplinary case conferences and clinical meetings.
- Present patient updates, identify barriers and recommend appropriate interventions.
- Coordinate services among nursing, therapy, social work, home health aides, physicians, pharmacies, DME providers and other community resources.
- Promote timely and effective communication among members of the care team.
- Serve as a clinical resource for field staff regarding assigned patients.
- Assist with resolving clinical, documentation and care-coordination issues.
Quality and Compliance
- Ensure services are delivered in accordance with physician orders, agency policies and applicable federal and California requirements.
- Participate in chart audits, quality improvement activities, infection-control initiatives and survey-readiness efforts.
- Identify documentation, service-delivery and compliance concerns and communicate them to clinical leadership.
- Support corrective actions and process improvements when clinical or operational deficiencies are identified.
- Safeguard protected health information and comply with HIPAA requirements.
- Complete required documentation and follow-up activities within established timeframes.
- Maintain professional boundaries and follow applicable standards of nursing practice.
- Perform other related duties consistent with the position’s clinical responsibilities.
Required Qualifications
- Current, active and unrestricted California Registered Nurse license.
- Minimum of two years of recent RN experience caring for high-acuity or medically complex patients.
- Strong clinical background in home health, intensive care, emergency care, step-down, telemetry, medical-surgical nursing, infusion therapy, complex wound care or a comparable setting.
- Demonstrated ability to assess changes in condition, identify clinical risks and determine when intervention or escalation is necessary.
- Ability to coordinate care for patients with multiple diagnoses, complex medication regimens, wound-care needs, infusion services or frequent hospitalizations.
- Ability to independently organize and manage an assigned caseload after completing required agency-specific orientation and onboarding.
- Current CPR/BLS certification.
- Valid driver’s license, reliable transportation and current automobile insurance.
- Strong clinical assessment, critical-thinking, organization and documentation skills.
- Ability to communicate professionally with patients, authorized representatives, physicians and interdisciplinary team members.
- Proficiency with electronic medical records and general computer applications.
- Ability to travel to patient homes within assigned service areas.
- Ability to perform the essential functions of the position with or without reasonable accommodation.
- Authorization to work in the United States.
Preferred Qualifications
- Previous home health or RN case-management experience.
- Experience with OASIS documentation, Plans of Care, recertifications and home health Conditions of Participation.
- Experience reviewing clinical documentation or conducting chart audits.
- Experience using WellSky/Kinnser or a comparable home health electronic medical record.
- Familiarity with Medicare, Medi-Cal and managed-care authorization requirements.
- Experience managing patients who require infusion therapy, complex wound care, disease management or post-hospitalization monitoring.
- Bilingual skills are helpful but not required.
Compensation
The expected hiring range is $50.00–$57.00 per hour. The final rate will be based on relevant RN experience, high-acuity clinical experience, home health and OASIS knowledge, qualifications and internal equity.
This is a nonexempt position and is eligible for overtime in accordance with applicable law. Employees must accurately record all time worked. Overtime should be approved in advance when practicable, but all overtime worked must be reported.
