Full Job Description

Job Summary

Join our dedicated healthcare team as a Home Health Aide / Certified Nursing Assistant (CNA), where you will play a vital role in delivering compassionate, personalized care to individuals in their homes and community settings. This position offers an exciting opportunity to make a meaningful difference by assisting clients with daily activities, providing essential medical support, and ensuring their comfort and safety. You will work closely with patients of all ages, including seniors, individuals with disabilities, and those receiving hospice or memory care, fostering independence and enhancing quality of life through your expertise and empathetic approach.

Responsibilities

Provide direct patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment like Hoyer lifts.
Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
Administer medications and perform catheter care following home healthcare regulations and physician instructions while maintaining patient confidentiality in compliance with HIPAA standards.
Support patients with specialized needs including Alzheimer’s care, dementia care, memory care, end-of-life care, and working with individuals with developmental disabilities or disabilities.
Assist with household tasks such as meal preparation, cooking, cleaning, laundry, and maintaining a safe environment to promote overall well-being.
Document patient interactions thoroughly in medical records and communicate effectively with healthcare teams to ensure continuity of care.
Employ patient transfer techniques safely during client movement or repositioning to prevent injury for both the patient and caregiver.

Requirements

Proven experience working as a Home Health Aide or CNA in settings such as assisted living facilities, nursing homes, hospice care, or home healthcare agencies.
Knowledge of medical terminology, medication administration protocols, vital signs measurement, catheter care, and basic life support (BLS) including CPR.
Familiarity with EMR/EHR systems for accurate documentation of patient information and medical records.
Ability to perform heavy lifting when necessary and operate equipment such as Hoyer lifts safely.
Strong understanding of home health care assistance regulations and compliance standards including HIPAA.
Excellent communication skills for effective client interaction and team collaboration.
Valid certification as a CNA or Home Health Aide; additional certifications such as First Aid and CPR are preferred.
Experience working with geriatric patients or individuals with disabilities is highly valued.
