IAFT/TFC Case Manager
Support Incorporated is seeking a dedicated and experienced Case Manager to join our Therapeutic Foster Care (TFC) and Intensive Alternative Family Treatment (IAFT) team.
This position is an opportunity to make a meaningful difference in the lives of children and adolescents who have experienced trauma, behavioral health challenges, and disruptions in their family or placement settings.
We are looking for someone who brings a strong work ethic, excellent communication and organizational skills, and the ability to work collaboratively with children, families, foster parents, schools, clinical providers, and community partners. The ideal candidate will have behavioral health case management experience and a genuine passion for helping children and families achieve stability, healing, and long-term success.
What You Will Do:
Coordinate Comprehensive Care
- Serve as the primary point of contact for children, families, therapeutic foster parents, service providers, and other members of the treatment team.
- Coordinate services among behavioral health providers, schools, social services, courts, medical professionals, and community resources.
- Help develop, implement, and monitor individualized treatment plans.
- Ensure that services and supports remain aligned with the child’s treatment goals and changing needs.
Assess Needs and Monitor Progress
- Assess each child’s strengths, needs, risk factors, supports, and barriers to success.
- Monitor progress through regular face-to-face visits, telephone contacts, team meetings, and communication with caregivers and providers.
- Identify concerns early and coordinate timely interventions when additional support is needed.
- Maintain clear documentation of progress, significant events, treatment updates, and changes in the child’s needs.
Support Therapeutic Foster Parents
- Provide weekly face-to-face supervision and support to therapeutic foster parents.
- Maintain daily contact with IAFT foster parents for check-ins, coaching, problem-solving, and crisis support.
- Help caregivers implement therapeutic strategies consistent with the IAFT model and the child’s treatment plan.
- Provide education, resources, and practical guidance to help caregivers respond effectively to trauma-related behaviors and emotional needs.
Advocate and Collaborate
- Advocate for the child and family in schools, courts, treatment meetings, and community settings.
- Participate actively in multidisciplinary team meetings, child and family team meetings, case reviews, and placement-related meetings.
- Work collaboratively with foster parents, biological families, guardians, DSS representatives, schools, providers, and other stakeholders.
- Help identify and address barriers that may interfere with treatment progress or placement stability.
Engage and Educate Families
- Promote meaningful involvement from biological families, guardians, and natural supports whenever appropriate.
- Help strengthen communication and relationships among children, families, and caregivers.
- Educate families and caregivers about trauma, behavioral health needs, treatment recommendations, and available resources.
- Support planning for reunification, permanency, placement stability, and successful transitions.
Maintain Documentation and Program Compliance
- Complete progress notes, treatment plans, assessments, reports, and other required documentation accurately and on time.
- Ensure compliance with agency policies, state regulations, funding requirements, and IAFT/TFC program standards.
- Track client progress and outcomes to support quality improvement and demonstrate program effectiveness.
- Work closely with NC Rapid Resource staff and follow applicable Mental Health Services Standards, Exceptional Children requirements, and Department of Public Instruction guidelines.
Minimum Qualifications
- Must meet the requirements of a Qualified Mental Health Professional.
- Bachelor’s degree in a human services field such as psychology, social work, counseling, sociology, or a closely related field is strongly preferred.
- Other degrees may be considered when accompanied by significant experience working with children, adolescents, families, or behavioral health populations.
- Minimum of two years of experience providing mental health or substance use services to children or adolescents.
- Significant case management experience within the human services or behavioral health field is required, with four years of relevant case management experience strongly preferred.
- Must be willing and able to complete required IAFT training and support the fidelity of the IAFT model.

Professional Development
The IAFT/TFC Case Manager will participate in regular supervision, required IAFT training, agency training, and ongoing professional development. The Case Manager is expected to remain informed about evidence-based practices, regulatory requirements, and emerging issues affecting children and families involved in therapeutic foster care.
