Location: Northeast Minneapolis, Shoreview, Columbia Heights, New Brighton, Hilltop, St. Anthony, Roseville, and surrounding communities
Schedule: Full-Time or Part-Time | Monday-Friday | No Weekends | No Holidays
Compensation: $70,000-$102,000 annually

More Independence Doesn't Have to Mean More Driving

One of the biggest misconceptions about home health is that you'll spend your entire day behind the wheel.

A well-built territory changes that.

Instead of sending you from one side of the Twin Cities to another, we work to organize patient visits geographically and build coverage around the communities our clinicians serve.

That means more of your workday can be spent providing therapy instead of getting to therapy.

You'll still have the freedom of being out in the community, but with a territory designed to make your day more manageable.

See Your Treatment Plan Work in Real Life

As a PTA, you'll implement plans of care established by the supervising Physical Therapist, but home health gives you a unique perspective on whether those interventions are actually working.

Is your patient navigating their stairs more safely?

Can they get out of their chair without assistance?

Are they consistently completing their home exercise program?

Has their balance improved enough to confidently move throughout their home?

You don't have to guess. You get to see it.

What You'll Be Doing

Provide Physical Therapy treatment visits in patient homes.
Implement individualized plans established by the supervising PT.
Provide gait training, strengthening, balance activities, ROM, mobility training, and functional exercises.
Reinforce home exercise programs.
Educate patients and caregivers on safety and mobility strategies.
Monitor patient response and functional progress.
Communicate clinical observations and changes to the supervising Physical Therapist.
Complete accurate and timely EMR documentation.
Collaborate with PTs, nurses, OTs, and other interdisciplinary clinicians.

Coming From Outpatient, SNF, or Rehab?

Your experience can transfer exceptionally well.

We're interested in PTAs coming from Outpatient Therapy, Skilled Nursing, Inpatient Rehabilitation, Transitional Care, Senior Living, Long-Term Care, Orthopedics, and Home Health.

Previous home health experience is preferred but isn't required.

We're looking for someone who is organized, dependable, comfortable working independently, and confident building relationships with patients and families.

Required Qualifications

Active Minnesota Physical Therapist Assistant license
Valid driver's license, automobile insurance, and reliable transportation
Strong patient communication skills
Ability to independently manage a field-based workday
Strong collaboration with the supervising Physical Therapist
Commitment to high-quality, patient-centered care

Preferred Qualifications

Home health experience
Experience with geriatric or medically complex patients
Strong interest in one-on-one rehabilitation
Comfort managing a territory-based schedule
