Full Job Description

We are hiring for a Care Coordinator position to work in our Assisted Living. This person would need to work at least 5 shifts per week Monday through Friday, with some weekend availability if needed. We would like to hire someone who has 2-4 years of CNA and Med Tech experience. This is a full time position.

Care Coordinator Responsibilities include:

-Manage and post CNA schedule weekly. Coordinate departmental schedule and ensure adequate staffing. Arrange for coverage of job duties within the department during employee absences either through delegation or personal completion of duties.
-Interview, hire, evaluate, coordinate, motivate, monitor performance, schedule and supervise department staff in accordance with the company policy.
- Communicating with resident family members to address concerns
- Train staff and help encourage teamwork
- Help make adjustments to resident care plans as needed

Requirements:
- Previous CNA experience required
- Caring, compassionate, and able to identify and meet the needs of others.
- Reliable, dependable, and punctual.
- Hard-working and team-building attitude.
- Excellent verbal and written communication skills.
