The Case Manager, Behavioral Health provides case management and support services through Coordination and Community Living Supports (CLS)/Respite programs for children, youth, and families. This role is responsible for ensuring quality care, coordinating services, advocating for clients, and supporting positive outcomes in accordance with all contractual, regulatory, and accreditation requirements.

Essential Duties and Responsibilities

Maintain primary responsibility for an assigned caseload, including intake, assessment, periodic reviews, annual assessments, case documentation, progress notes, collateral contacts, and case file management.
Provide comprehensive case management services, including social casework, engagement, mentoring, advocacy, and community referrals for youth ages 3–21.
Collaborate with Clinical Supervisors, Program Managers, families, and other treatment team members to develop, implement, and evaluate Person-Centered Plans.
Promote the health, safety, and well-being of individuals served while supporting them in the most integrated and independent community setting possible.
Assist individuals with daily living activities and skill development, including communication, household tasks, recreation, and community participation.
Support families in caring for children, attending appointments, securing resources, providing transportation, and assisting with medication administration and non-medical care as needed.
Coordinate services with community mental health agencies, healthcare providers, schools, referral sources, and other community partners.
Maintain at least monthly communication with each child's mental health provider to monitor treatment progress and ensure service coordination.
Partner with educational systems and schools to support treatment plan goals and educational success.
Assist youth and families in accessing resources, benefits, and community services, including support with guardianship processes when applicable.
Respond effectively to crisis situations using appropriate intervention and de-escalation techniques.

Qualifications

Education

Bachelor's degree in social work, Psychology, Human Services, Counseling, or a related human services field required.
Master's degree preferred.

Experience

One year of experience working with children, individuals with disabilities, and/or individuals with mental illness preferred.
Experience in residential, behavioral health, case management, or community-based services preferred.

Knowledge, Skills, and Abilities

Strong crisis intervention, problem-solving, and de-escalation skills.
Excellent organizational, time management, and prioritization abilities.
Proficiency with computers and software applications, including spreadsheets and electronic documentation systems.
Demonstrated compassion, empathy, and a non-judgmental approach when working with vulnerable populations.
Ability to engage effectively with children and families experiencing multiple challenges.
Experience collaborating with multidisciplinary treatment teams and developing outcome-based service plans.
Strong verbal and written communication skills.
Ability to work professionally with families, service providers, schools, community partners, and regulatory agencies.
Cultural competency and the ability to effectively serve diverse populations.

Additional Requirements

Valid, unrestricted driver's license with a satisfactory driving record.
Reliable personal transportation suitable for transporting individuals served.
Proof of current automobile insurance.
