Full Job Description

Position Summary

A brand-new skilled nursing and rehabilitation facility is opening in Johns Creek, GA, and we are seeking an experienced Director of MDS (MDS Coordinator) to build and lead our resident assessment program from the ground up. Reporting to the Director of Nursing (DON), you will own the Resident Assessment Instrument (RAI) process, drive accurate and timely MDS 3.0 assessments, and safeguard the reimbursement, compliance, and quality outcomes that a new nursing home depends on. This is a registered nurse (RN) leadership role at the intersection of clinical care, documentation, and revenue integrity.

We will be honest that opening a new building is demanding work, and the MDS function is the financial and regulatory backbone of it. In return, you get a rare ground-floor opportunity: you will build the assessment workflows, the triple-check process, and the interdisciplinary rhythm from day one, with real support from nursing leadership, ownership, and operations as our census grows. If you are a detail-driven MDS leader who wants to set the standard rather than inherit someone else’s, this is the role.

Key Responsibilities

● Coordinate and oversee the entire RAI process, ensuring accurate, complete, and timely completion of all MDS 3.0 assessments (admission, quarterly, annual, significant change, and Medicare-required assessments).

● Set and manage Assessment Reference Dates (ARDs) to optimize appropriate reimbursement under PDPM and Medicare Part A, and coordinate Medicaid case-mix assessments as required in Georgia.

● Complete Care Area Assessments (CAAs) and lead the interdisciplinary team (IDT) in developing and updating individualized, resident-centered care plans.

● Ensure timely and accurate electronic submission of MDS data to CMS (iQIES) and reconcile validation reports and error corrections.

● Lead the triple-check process with billing, therapy, and nursing to verify claim accuracy before submission.

● Monitor and improve Quality Measures (QMs), Five-Star ratings, and CASPER reports, and support survey and audit readiness.

● Audit supporting documentation in the medical record to validate MDS coding and defend reimbursement in the event of ADRs or audits.

● Educate and mentor nursing and interdisciplinary staff on accurate documentation, coding, and RAI requirements.

● Track assessment schedules and maintain compliance with all state, federal, and CMS regulations and facility policies.

● Partner with the DON and Administrator on reimbursement strategy, census development, and clinical quality as the building grows.

Required Qualifications

● Active, unrestricted Registered Nurse (RN) license in the state of Georgia through the Georgia Board of Nursing.

● Graduate of an accredited nursing program (ADN or BSN).

● Working knowledge of the RAI process, MDS 3.0, Care Area Assessments (CAAs), and interdisciplinary care planning.

● Understanding of PDPM, Medicare Part A/B, and Medicaid case-mix reimbursement in a skilled nursing facility (SNF) / long-term care (LTC) setting.

● Current BLS / CPR certification.

● Proficiency with electronic medical records (EMR) and MDS software.

● Strong organizational, analytical, and communication skills with meticulous attention to detail and deadlines.

● Ability to pass a pre-employment background check, fingerprinting, drug screen, TB test, and required immunizations in accordance with long-term care requirements.

Preferred Qualifications

● RAC-CT certification (Resident Assessment Coordinator – Certified) or willingness to obtain.

● 2+ years of prior MDS Coordinator or MDS Director experience in a nursing home or SNF.

● Experience opening or building an MDS/assessment program in a new facility.

● Hands-on experience with PointClickCare, or similar clinical/MDS platforms.

● Knowledge of Georgia Medicaid case-mix and PDPM optimization strategies.

● Prior charge nurse, unit manager, or nursing leadership experience.

Schedule and Location

This is a full-time, salaried leadership position, generally Monday through Friday during day shift, with the flexibility a new building requires around assessment deadlines and month-end. You will be on-site at our new facility. This is an on-site role, not remote.

Physical Requirements

● Ability to sit and work at a computer for extended periods completing assessments and documentation.

● Ability to move throughout the building to observe residents, review charts, and meet with the interdisciplinary team.

● Ability to stand, walk, bend, and occasionally lift or support up to 25 pounds.

● Visual, auditory, and cognitive acuity sufficient to assess residents and analyze detailed clinical data.

● Ability to wear personal protective equipment (PPE) per infection-control protocols.
