Full Job Description

Job Overview

Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of patients. In this vital role, you will provide personalized assistance to individuals in their homes, supporting their health, safety, and comfort. Your energetic approach and commitment to quality patient care will help promote independence and well-being for those in need of home health services. This position offers an exciting opportunity to work closely with diverse populations, including geriatric patients, individuals with disabilities, and those receiving hospice or memory care.

Duties

Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts.
Monitor and observe patient conditions, including vital signs, and report changes promptly to healthcare professionals.
Administer medications and perform catheter care following medical directives and home healthcare regulations.
Support patients with meal preparation, light housekeeping, laundry, and maintaining a clean and safe environment.
Document patient care activities accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while maintaining strict confidentiality in accordance with HIPAA standards.
Provide specialized dementia and Alzheimer’s care, memory care, hospice support, and end-of-life assistance with compassion and professionalism.
Communicate effectively with clients and their families to ensure personalized care plans are followed and needs are met.

Requirements

Proven experience working as a Home Health Aide or in assisted living, nursing homes, or long-term care environments.
Knowledge of medical terminology, medical documentation, medication administration, and home health care protocols.
Certification in First Aid and Basic Life Support (BLS), including CPR training.
Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts.
Familiarity with EMR/EHR systems for accurate recordkeeping; experience working with clients who have developmental disabilities or geriatrics is highly valued.
Strong communication skills for effective client interaction and team collaboration.
Valid driver’s license and reliable transportation preferred for client visits.
