Full Job Description

Job Title: Registered Nurse (RN) – Field/Home Health

Job Description: We are seeking an experienced and compassionate Registered Nurse (RN) to join our home health team with a primary focus on OASIS assessments, including Start of Care (SOC), Resumption of Care (ROC), and Recertification visits. This role will also provide routine skilled nursing visits as needed to support continuity of patient care.

Qualifications:

Current RN license in good standing.
Home health experience preferred.
OASIS documentation experience strongly preferred.
Excellent assessment and communication skills.
Valid driver's license, reliable vehicle, and auto insurance.
Ability to work independently and efficiently in the field.

Essential Job Functions:

Evaluate potential clients and maintain contact with primary care physicians for each client, for medical information, physician clearance and continuity of care.
Maintain an ongoing assessment of all clients within your care and adjust client care plan as necessary with the involvement of the client's multidisciplinary team i.e. PCP, Therapist etc.
Plan, implement and evaluate all nursing interventions to ensure client safety and positive outcomes
Maintain effective communication with all members of the multidisciplinary team to ensure positive outcomes and client safety i.e. PCP, Therapist, Pharmacist, Secondary Nurse Etc.
Ensure all intake paperwork, notes and Oasis' are turned into the office within 24 hours after completion of the appointment.
Ensure all medications are picked up from pharmacy at the proper rotation and clients medications are reconciled with MD orders.
Perform Narcotic Count with Pharmacy during time of Pick up and when reporting off client to another nurse if this change occurs.
Provide daily notes for all visits, must be submitted within 24 hours.
Conduct review for appropriate utilization of services from admission to discharge
Advocate effectively for the clients we serve.
Primary nurse will complete the weekly review documentation report to be handed into the clinical manager to give a brief synopsis on each client.

Preparation, Knowledge, skills and Abilities

Graduate of an Accredited School of Professional Nursing.
Currently Licensed as a RN through the State Board of Nursing Massachusetts
At least one year of nursing experience in the homecare field Possesses and maintains current CPR certification
Have excellent observation skills, good nursing judgment with excellent written and oral communication skills.
Has good organizational skills
Is flexible and cooperative while fulfilling obligations
Must possess a functional mobile phone and vehicle at all times.

Working Conditions/Physical Demands:

Able to assess clients health status using visual inspection and observation to detect changes in physical appearance
Ability to accurately read labels on medications

Pay: $50.00 - $70.00 per hour
