Full Job Description

About us:

Cornerstone Health systems is a family-owned medical equipment company in Hohenwald, TN. We are a company with integrity full of tireless workers who know how to work effectively. What you’ll do:

As a hospice logistics representative you will coordinate with hospices to input orders across multiple platforms for hospices across Tennessee. You will also speak with patients to update them about times for deliveries and pickups.

In Depth:

· Participate in Improvement of Operational Performance (IOP) activities

· Participate in Hospice Team EOS Meetings

· Patient rights and HIPAA education

· Create New Patients

· Input Sales Orders into Brightree and send POD to Transport in a timely manner

· Create Pickup Orders and send POD

· Build, adjust, and optimize Routes in Transport Manager and TMA (DME Track) and assign to Medical Equipment Technicians (METs) while minimizing overtime

· Follow the Route Adjustment Decision Guide to help ensure efficiency, reduce overtime, reduce skipped/ repeated stops and document changes

· Call patient families to coordinate pickups using Company provided scripts

· Call patient families to coordinate deliveries using Company provided scripts

· Communicate with Hospices on deliveries and pickups

· Create and maintain notes in all systems in real time (Brightree, DME Track, and Qualis)

· Communicate with METs about their routes and adjustments made

· Actively monitor MET routes through Fleetraks and adjust as necessary, minimizing overtime

· Actively monitor TMA completion and communicate with METs

· Reconcile orders from Transport Manager and notate new information in DME Track and Brightree

· Monitor and note lost equipment claims accurately

· Maintain Scorecard and efficiency numbers

· Coordinate with hospices on live discharges and gather documentation and payment information

· Communicate with office team members about live discharges, lost equipment, and important updates throughout the day

· Route efficiency, minimized skipped stops and repeated stops

· Patient and Hospice Satisfaction

Your qualifications:

· High school diploma or general education degree (GED)

· Strong organization and multitasking skills

· Logistics, dispatch, or customer service experience

· Detail oriented

· Excellent phone and written communication

Pay: $16.00 - $19.00 per hour
