Full Job Description

Holistic Consultants

Holistic Consultants is looking to hire a highly motivated, team player who enjoys caring for others. We are looking for additional trained staff to join our team. Must be willing to work with Clients/Patients with Special needs which can be mental health and intellectual disabilities.

This position is under the direct supervision of the Administrator and the House Supervisor when the Administrator is not on the premises. The position includes the provision of residential treatment as well as socialization, educational and recreational activities for developmentally disabled consumers who manifest a variety of emotional and behavioral disorders.

These services are provided within a framework consistent with other treatment goals under the overall supervision of the Administrator. CNA/Direct Care Staff work in conjunction with other residential staff, as well as other persons/agencies involved in treatment activities related to the client's Individual Service Plan (ISP).

Direct Care Staff directly assists residential consumers in:

1. Activity of Daily Living

2. Health care monitoring/management as directed by RN, LVN and Primary care Provider

3. Socialization and integration through activities and learning activities

4. Developing the necessary living skills required for residential environments.

5. Resolving emotional and interpersonal problems that directly affect residential living.

6. Developing interpersonal skills within a supportive residential environment.

7. Safely transporting clients to appointments, community activities, and other approved destinations, including being comfortable using personal vehicle when required in accordance with agency policy.

Duties and Responsibilities:

To develop positive and supportive relationships with consumers and also interdisciplinary staff

· Observing, recording, and evaluating the behavior of consumers.

· Providing appropriate training/education

o Cooking, hygiene, budgeting, and other relevant living skills.

Assist clients that are ambulatory and non-ambulatory in there ADL’s.

Communication, functional living skills, social skills, self-help skills, and recreational and leisure skills

· Providing socialization, education, and recreational activities consistent with the goals of residential living.

· Evaluating emergencies and taking actions that promote the well-being of consumers, other staff and the community.

To be responsible for behavioral program training to ensure the safety and well-being of consumers.
To develop data collection systems needed to document consumer’s progress.
To participate in clinical meetings to review consumer’s progress.
To conduct Functional Behavior Assessments.
To prepare the consumer’s initial 30-Day Behavior Assessment and Intervention Plans.
To prepare the consumer’s quarterly progress reports.
To attend, actively participate in, and conduct on-going trainings, in-services sessions, and other meetings as scheduled.
To oversee the members of the clinical team.

· Evaluating emergencies and taking actions that promote the well-being of consumers, other staff and the community.

· Planning activities in conjunction with the Administrator and other residential staff.

To review Treatment Integrity data as well as monthly behavioral data trends.

· Consulting with other agency personnel, such as Case Managers, when necessary to enhance client and program progress.

Minimal Qualifications

· Must be at least 21 years of age.

· Minimum of six months prior experience providing direct supervision and special services; or

o Within six months of beginning to provide direct supervision and special services in the facility, complete at least 12 additional hours of related continuing education.

o DSP 1 and DSP 2 enrolled or completed

· Completion of a minimum of 12 hours continuing education meeting Title 17

· Ability to express self clearly and concisely in the English language, orally and in written form.

· Successful completion of competency-based training (Direct Support Professional) and passing of the competency test required for a direct care staff for the year in which the training is satisfactorily completed. (Can be enrolled but must pass the Competency test)

· Requirements within each 12-month period following the assumption of direct care staff duties.

· Completion of Criminal Record Statement, Fingerprint clearance and clean DOJ and FBI background check

· Health screening and the necessary physical qualifications to perform duties as assigned.

· First Aid and CPR certification required.

Willing to receive additional and on-going training for the position
Health screening (mandatory)

Good standing California driving record.

MUST have Driver's License
