We are seeking a behavioral health professional with experience supporting patients living with chronic medical conditions. This position offers the opportunity to work collaboratively with primary care providers, specialists, psychiatrists, and therapists to improve patient outcomes and quality of life.
Position Overview
We are seeking a licensed psychologist, clinical social worker, marriage and family therapist, professional clinical counselor, or other qualified behavioral health provider to deliver evidence-based behavioral health interventions for patients with chronic medical illnesses.
This fully remote role focuses on helping patients manage the psychological, behavioral, and lifestyle challenges associated with chronic disease. Common patient populations may include individuals living with chronic pain, diabetes, obesity, cardiovascular disease, cancer, gastrointestinal disorders, sleep disorders, long-term disability, and organ transplantation.
The ideal candidate has experience working within integrated healthcare settings and understands the complex relationship between physical and behavioral health.
Responsibilities
- Provide behavioral health treatment and counseling via telehealth.
- Support patients coping with chronic medical conditions, including:
- Chronic pain
- Obesity and weight-related conditions
- Diabetes
- Heart disease
- Cancer
- Sleep disorders and insomnia
- Gastrointestinal disorders
- Long-term disability
- Organ transplantation
- Deliver evidence-based interventions to improve health outcomes, treatment adherence, and quality of life.
- Provide Cognitive Behavioral Therapy for Insomnia (CBT-I) and other sleep-focused behavioral interventions.
- Provide behavioral interventions related to weight management, emotional eating, lifestyle change, and long-term weight maintenance.
- Support patients using weight-loss medications and lifestyle medicine programs.
- Assist patients with adjustment to diagnosis, illness-related stress, anxiety, depression, and medical trauma.
- Collaborate closely with primary care providers, specialists, psychiatrists, and other members of the care team.
- Maintain timely and accurate clinical documentation within the electronic health record.
- Participate in multidisciplinary case reviews and care coordination activities.
QualificationsRequired
- Active California licensure as one of the following:
- Psychologist (PhD or PsyD)
- Experience providing psychotherapy or behavioral health treatment.
- Strong communication and interdisciplinary collaboration skills.
- Comfortable working in a fully remote environment.
Preferred
- Experience in health psychology, behavioral medicine, integrated primary care, lifestyle medicine, or medical social work.
- Experience working with patients living with chronic medical conditions.
- Training or experience in Cognitive Behavioral Therapy for Insomnia (CBT-I).
- Experience supporting patients with obesity, weight management, or lifestyle behavior change.
- Training in evidence-based approaches such as:
- Cognitive Behavioral Therapy (CBT)
- Acceptance and Commitment Therapy (ACT)
- Motivational Interviewing (MI)
- Behavioral activation
- Chronic pain management interventions
- Behavioral weight management strategies
- Experience collaborating with medical providers in an integrated healthcare setting.
