Full Job Description

The Chronic Care Management (CCM) Case Manager is responsible for coordinating and managing the care of patients with multiple chronic conditions through proactive outreach, care coordination, patient education, and ongoing communication with providers, specialists, facilities, and caregivers. This position plays a vital role in improving patient outcomes, reducing hospitalizations, enhancing quality of care, and supporting value-based care initiatives while maintaining compliance with Medicare CCM requirements.

Essential Duties and Responsibilities

Manage a panel of patients enrolled in the Chronic Care Management (CCM) Program.
Complete and document all required monthly CCM services in accordance with Medicare guidelines.
Perform comprehensive patient assessments and develop individualized care plans in collaboration with the patient's primary provider.
Conduct monthly patient outreach by telephone or other approved communication methods to assess health status, medication adherence, and care needs.
Coordinate care among physicians, specialists, hospitals, skilled nursing facilities, home health agencies, hospice providers, pharmacies, and community resources.
Educate patients and caregivers regarding chronic disease management, medications, preventive care, and available healthcare resources.
Identify barriers to care and assist patients in accessing appropriate medical, behavioral health, social, and community services.
Monitor hospital admissions, emergency department visits, and discharges to facilitate timely follow-up and reduce readmissions.
Review medications for adherence concerns and communicate potential issues to the clinical team.
Escalate urgent clinical concerns to the supervising provider for evaluation and intervention.
Maintain accurate, timely, and complete documentation within the electronic health record (EHR).
Ensure all CCM documentation supports billing and complies with CMS regulations and company policies.
Assist with Annual Wellness Visit (AWV) coordination and other preventive care initiatives as assigned.
Collaborate with providers and office staff to identify eligible patients for CCM enrollment.
Obtain and document patient consent for participation in the CCM Program.
Maintain patient confidentiality and comply with all HIPAA privacy and security regulations.
Participate in quality improvement initiatives, population health programs, and value-based care activities.
Attend departmental meetings and complete required training and continuing education.
Perform additional duties and special projects as assigned by the Director of Care Management, Director of Operations, or Chief Operating Officer.

Required Qualifications

Current licensure as a Registered Nurse (RN), Licensed Practical Nurse (LPN), Medical Assistant (MA), Social Worker, or other qualified healthcare professional, as permitted by CMS guidelines and state regulations.
Minimum of one (1) year of clinical or case management experience preferred.
Experience with chronic disease management, population health, or care coordination preferred.
Knowledge of Medicare Chronic Care Management (CCM) guidelines preferred.
Strong communication, patient education, and interpersonal skills.
Excellent organizational, documentation, and time management abilities.
Ability to manage multiple patient cases while maintaining attention to detail.
Proficiency with electronic health records (EHR), Microsoft Office, and care management software.

Pay: $23.00 - $30.00 per hour
