Your Schedule

Part-Time [9am - 5pm] Tuesday, Wednesday, Thursday + 1 Weekend On Call Per Month
Full Time Available

Compensation

$30.00-$35.00 per hour, based on experience and qualifications.

Position Overview

As our RN Case Manager, you'll oversee the quality and coordination of care for clients receiving services in their homes. This position averages 3-4 client assessments or supervisory visits per day, allowing time to provide thorough evaluations, documentation, and caregiver support.

Primary Responsibilities

Complete in-home nursing assessments and routine supervisory visits.
Evaluate client needs and ensure care plans remain appropriate and up to date.
Monitor the quality of services provided by caregivers.
Review documentation and maintain accurate electronic records.
Provide clinical guidance, education, and oversight to caregivers.
Collaborate with office staff to coordinate exceptional client care.

Qualifications

Current Registered Nurse (RN) license (Virginia or Compact State License).
20+ years of nursing experience preferred (experienced clinicians are strongly encouraged to apply).
Stable work history with demonstrated professional commitment.
Valid driver's license, dependable transportation, and current auto insurance.
Comfortable traveling locally for home visits.
