Social Services Coordinator

Reports To: Administrator

Department: 600

FLSA Status: Exempt

General Description

Essential Duties

Works with residents, family’s members and representatives toward adjustment to skilled nursing facility living, crisis of living, deaths, disabilities, etc.
Appropriately documents social service related information in residents charts as needed.
Coordinates, distributes and communicates changes to the monthly care conference schedule. Send out invitations to residents, family members and other relevant parties.
Maintains and communicates changes in facility census and distributes to other departments on a regular basis.
Orders adaptive and other types of clothing and personal goods for residents as requested by residents, family members, staff, and others.
Works to establish continually nurture a positive working relationship with residents, family members, staff and others.
Assists in the formulation of policies and procedures for the facility.
Serve as the primary admission coordinator, screening the needs of admission applicants in a timely and efficient manner. This includes working with the interdisciplinary team to screen the potential residents, contacting relevant parties and coordinating any miscellaneous services that are indicated.
Serves as primary Discharge Planning Coordinator, integrating discharge planning for residents transferring to another facility, assisted, independent or alternate group living. Makes referrals to outside agencies in coordinating discharge services on an as needed basis.
Coordinates rooms changes, whenever possible, per resident or family request or staff recommendation. Completes all necessary documentation related to room changes.
Effectively identifies social, emotional, spiritual and psychological issues and applies problem solving skills to resolve or minimize the effects of these issues on residents.
Assist in planning and maintaining social services budget.
Practices social work in an ethical and professional manner, as outlined by the social work code of ethics. Abides by all social work licensure guidelines and maintains appropriate educational and professional affiliations.
Is approachable, knowledgeable and friendly to all customers, including residents, family members, representatives, co-workers and others.
Assist residents In maintaining independence with financial matters, including assisting with managing checkbooks, sorting mail, mailing bills, filling out insurance paperwork, securing alternative sources of funding and answering questions on an as needed basis.
Provides no less than annual education for staff regarding the vulnerable adult act, resident rights, and advanced directive information and other social service related topics as requested by staff.
Attends and participates in scheduled meetings, including, but not limited psych drug, resident council, PPS, care conferences, manager meeting and other meetings as assigned.
Responsible for the overall investigation and follow up of the VA policy and Grievance policy.
Responsible for the oversight of the beauty salon.
Accurately completes a social history, MDS assessment and performs resident interview, as well as other relevant assessments and care plan for resident in required time frames.

Other Duties

Presents a positive public image to residents, families, staff, physicians, and visitors. Contributes to team cohesiveness, promotes new ideas, respects peers and creates a pleasant work atmosphere.
Attends mandatory in-services as required. Attends all other in-services as requested by department manager or administrator.
Supports the concepts and ideas of culture change/resident centered care. Promotes individual resident choices, routines, and preferences.
Maintains confidentiality of resident related information In compliance with HIPAA regulations. Respects confidentiality of resident related information in compliance with HIPAA regulations. Respects confidentiality of the organization, department, staff, residents, and families.
Accepts assignments designated by supervisor in a positive manner. Id flexible in changing assignments with little notice to accommodate residents and facility needs.
Adheres to all safety policies and procedures. Responds appropriately to fire, tornado, and disaster alerts and drills. Follows facility and department safety environmental and infection control standards.
Adheres to dress code. Is neat and clean in appearance. Wears nametag at all times while on duty. Follows the facility/department dress code.
Other duties that may be assigned by the department manager/administrator.
Gives proper notice of tardiness or absences. Adheres to meal/break times. Accomplishes workload within time allowed

Minimum Experience and Qualifications

Education:

Bachelors in social work from an accredited college or university.
Valid Minnesota social work license.

Experience:

2 years(s) preferred in Nursing home social work experience preferred but not required.
