Job Title: Primary Care Nurse Practitioner (NP) | Community-Based Care

Location: Willmar, Minnesota & Surrounding Communities

Compensation: $120,000 - $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

You'll become the provider who understands their medical history, recognizes subtle changes in condition, collaborates closely with nursing staff, and helps families navigate important healthcare decisions.

What You'll Be Doing

Provide comprehensive primary care services throughout assisted living, memory care, independent living, and skilled nursing communities.
Diagnose, treat, and manage acute illnesses and chronic medical conditions.
Perform comprehensive assessments, annual wellness visits, and routine follow-up care.
Develop individualized treatment plans focused on improving long-term patient outcomes.
Manage chronic conditions and monitor changes in patient health over time.
Coordinate care following hospitalizations and specialty consultations to help reduce avoidable readmissions.
Collaborate with physicians, nurses, therapists, pharmacists, social workers, caregivers, and senior living leadership.
Educate patients, families, and caregivers while serving as a trusted clinical resource.
Communicate changes in condition and treatment plans clearly with the patient's broader care team.
Identify opportunities to improve quality measures, patient satisfaction, and continuity of care.

Required Qualifications

Active Minnesota Nurse Practitioner (NP) license.
Strong assessment, diagnostic, and clinical decision-making skills.
Ability to practice independently while collaborating within a physician-led team.
Strong communication and care coordination skills.
Passion for caring for older adults.
Reliable transportation and willingness to travel throughout Willmar and the surrounding communities.

Preferred Qualifications

Experience managing medically complex seniors.
Chronic disease management experience.
Primary care, geriatric, or community-based clinical experience.
Assisted living, memory care, or skilled nursing experience.
Wound Care Certification (WCC) is a plus.
