Job Summary
This position is responsible for the day-to-day operation of clinic activities. The Site Supervisor will oversee administrative and clinical support staff, coordinate patient flow, ensure regulatory compliance, and resolve on-site issues to maintain a high standard of patient care. Responsibilities also include assisting with hiring, evaluating staff, maintaining schedules and assigning daily work. This position will also provide direct patient care and back-up coverage as needed, facilitate provider/patient interaction, and represent the clinic in organizational projects and committees and community events.

- Supervise clinic operations in accordance with the organization's policies and applicable laws.
- Responsibilities include assistance with interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
- Coordinates team-based care to provide health services to individuals through effective partnerships with patients, their caregivers/families, community resources, and their physician. Facilitates a “shared goal model” within and across settings to achieve coordinated high-quality care that is patient and family centered.
- Assist with providing a strategic approach to detect early and manage effectively the chronically ill patient population.
- Participate in all Care Coordination related to Clinic Patients. Participate in all training and meeting activities.
- Provides safe and therapeutic anticoagulation instruction and patient education.
- Supervise staff to ensure that all health care practices follow legal and ethical regulations and clinic policies.
- Maintain appropriate daily staff schedule. Obtain coverage for sick call and planned absences.
- Ensure efficient patient flow and proper scheduling, and adjust assignments as needed.
- Carries out orientation and in-service programs; and other staff development activities for clinical and support staff.
- Supervises the performance of assigned staff including coaching, corrective action, and recognition.
- Processes payroll for clinic staff as assigned.
- Prepares agenda and leads monthly staff meetings to identify and resolve problems and to disseminate information.
- Communicate directly with the Chief Nursing Officer or designee regarding staff performance, clinic function, and quality of care on a regular basis.
- Ensure safe compliance with clinic safety and infection control policies and procedures and compliance with Federal, State, and local laws and regulations.
- Oversee staff assignments regarding clinic stocking, cleanliness, and creating an organized and safe work environment.
- Serve as a clinical resource and professional role model for all staff.
- Oversees clinic inventory and requisition of supplies and equipment.
- Provides direct or indirect patient care services, including, but not limited to, the administration of medications and therapeutic agents necessary to implement a treatment, disease prevention or rehabilitative regimen ordered by physician.
- Serves as support to other clinic as needed including but not limited to front desk coverage, lab draws, rooming patients, assisting with shots, etc.
- Consult with providers on disposition of walk-in patients as needed.
- Assist providers with emergency medical incidents.
- Serves as a strong patient advocate and provides direct patient or community education as necessary.
- Assists with provider referrals to outside consultants and referral sources.
- Ensures proper dispensing and secure storage of medications.
- Assists with and follows RN refill protocols.
- Performs procedures, tests, and treatments when needed.
- Respond to patient and/or staff complaints regarding clinic services.
- Carries out clinic policy and procedures and assists with revisions of same.
- Sets up MAW training for clinic staff, provides training and works on MAW reports required for clinics.
- Participates in ongoing quality assessment and improvement activities as assigned, documents information enabling data collection and root cause analysis needed to identify opportunities for improvement.
- May participate in committees and task forces.
- Represents the clinic in community health activities and events.

- Pursues programs of continuing education consistent with requirements of Minnesota Board of Nursing.
- Participates in educational conferences and in-services, and maintains professional knowledge and skills related to the areas of responsibility.
- Understands, accepts, and abides by the code of professional nurses.

Minimum Qualifications
- Associate's degree (A. A.) or equivalent from an accredited school of nursing, BSN preferred.
- Minimum three years of experience in primary care clinic setting.
- Previous experience in caring for chronic disease patients required.

- Ability to respond quickly and decisively to emergency situations and participate in shelter- in- place and disaster response protocols
- Ability to work with interdisciplinary teams, communicating effectively with patients, families, and healthcare providers
- Ability to demonstrate the knowledge and skills essential for providing care to specific patient populations
- Ability to lift, move, and position patients and equipment
Preferred Qualifications
- Supervisory and Triage experience preferred.
- Thorough knowledge of the principles and practices of primary care nursing or community health settings preferred.
- Previous experience with mobilizing community resources, navigating patients through the healthcare continuum, and working with disparate populations preferred.
- Previous experience in a rural, community hospital/clinic setting.
- Knowledge of CAH-specific regulations and rural health challenges.
Licenses & Certifications
- Maintains license as RN in the state of Minnesota.
- Maintains current AED/CPR certification.
Computer Skills
Proficiency with Microsoft Office Suite (Word, Excel, Outlook), secure messaging applications, and collaborative tools (e.g.) Microsoft Teams, Google Workspace. Proficient with Paycom and other Information Systems applicable to position. Ability to thoroughly document clinical services, maintain accurate patient records, and effectively utilize electronic health record (EHR) systems i.e. EPIC.

Mathematics & Analytical Abilities
Ability to accurately perform basic mathematical calculations in all units of measure using whole numbers, common fractions, percentages, ratios, and proportions to practical situations and in analysis of quality assurance data. Ability to understand bar graphs and other visual representations of data. Accurately record and interpret clinical measurements. Use EMR systems and care documentation tools to analyze data trends and support treatment.
