Pay based on experience (hourly)
M-F Dayshift with on-Call Rotation
$3 afternoon shift differential
$1.25 midnight shift differential
$2.00 weekend/holiday shift differential

Job Summary

The Resident Care Coordinator supervises, coordinates, and evaluates the clinical care needs of residents to ensure the delivery of safe, compassionate, and high-quality care. Under the direction of the Director of Resident Care and the Administrator, this position provides leadership to nursing staff, oversees resident care planning, coordinates with physicians and families, and ensures compliance with applicable regulations and organizational standards.

Duties

Resident Assessment & Clinical Care

Assesses and evaluates medical, social, psychological and spiritual needs of the resident.
Reviews assessment and information provided by LPN and Personal Care Attendants. Initiates or directs nursing action as is necessary.
Makes rounds to assigned facility periodically; evaluates new admissions, residents with signs/symptoms of illness, residents with special problems, or upon request of nursing staff.
Evaluates emergencies, makes independent nursing decisions based on good judgment, and implements actions accordingly.
Responds to all emergency stat calls within the facility.
Ensures that care is delivered to each resident in accordance with quality standards.
Ensures the provision of safe, economical and efficient nursing care.

Care Planning & Documentation

Reviews resident medical records to assist in resolving problems/needs and documents actions taken.
Maintains resident medical records, including nursing observations, medications, treatments, reactions, tests, intake and output, temperature, pulse, and respiration.
Records nursing needs of residents on the nursing care plan to assure continuity of care.
Supervises and assists in the development of resident support plans; supervises support plan conferences; updates support plans; informs nursing staff of the plan of care.
Supervises and assists with or completes paperwork requirements to provide resident care or meet regulatory requirements.
Maintains accurate, complete, and timely records according to policies and procedures.
Maintains confidentiality of information.

Physician, Resident & Family Communication

Acts as a liaison between physicians and nursing staff.
Ensures physicians are updated on residents' conditions and needs.
Ensures physician orders are obtained and carried out.
Acts as a liaison between residents, families, and physicians.
Informs families of changes in residents' conditions, untoward events, or changes in the plan of care, as appropriate.
Assists with resident and family teaching and counseling.
Participates in discharge planning and coordination of continuum of care in alternative settings.

Clinical Procedures & Medication Administration

Supervises and assists LPNs or physicians with examinations, treatments, special tests, or procedures; performs these independently as permitted under the Nurse Practice Act.
Prepares and administers medications, injections, colostomy care, and other treatments as permitted under the Nurse Practice Act.
Supervises verification that laboratory, x-ray, and other hospital reports are available and filed in the resident chart.

Staff Supervision, Training & Leadership

Plans, supervises, trains, and assists LPNs and Personal Care Attendants.
Supervises and assists in the orientation of new Personal Care Attendants, Medication Administration Class, competencies, and new procedures and equipment.
Demonstrates nursing techniques and procedures and supervises nonprofessional nursing personnel in rendering care.
Assigns and directs resident care to LPNs and Personal Care Attendants.
Provides supervision and leadership for LPNs and Personal Care Attendants.
Assists in the completion of annual performance evaluations.
Serves as a resource person to staff in solving clinical and resident care problems.
Attends and may participate in in-service education programs, including staff training.
Participates in nursing and facility meetings.

Admissions, Discharges & Resident Transitions

Supervises new admissions, discharges, and resident transfers, including assessments and completion of required documentation.
Participates in discharge planning and coordination of continuum of care in alternative settings.

Administrative & Management Responsibilities

Communicates information regarding policies, procedures, and other organizational communications; monitors compliance and provides feedback from staff to the administrator.
Listens to, assesses, and initiates action regarding personnel complaints or grievances and refers concerns to the administrator as necessary.
Interprets and implements principles of management.

QUALIFICATIONS NECESSARY TO PERFORM ESSENTIAL DUTIES AND RESPONSIBILITIES:

Must be a graduate from an accredited school of nursing and current licensure by the PA State Board of Nursing. Must successfully complete state approved Medication Administration Train the Trainer Course. Must be able to lift, turn and reposition residents both alone and with assistance with and without the use of mechanical devices. Must also be able to assist others with lifting, turning and repositioning residents both with and without the use of mechanical devices. Must be able to perform the essential functions of the position without posing a direct threat of harm to themselves, others or property. Must adhere to St. Paul’s personnel policies and practices including, but not limited to, the attendance requirements.
