Full Job Description

Job Summary
We are seeking a dynamic and compassionate Licensed Vocational Nurse (LVN) to join our team as a Field Nurse, delivering high-quality patient care directly in the community. In this vital role, you will provide comprehensive nursing services to individuals across various settings, including home care, assisted living, and long-term care facilities. Your expertise will help ensure patients receive personalized, effective treatment while promoting independence and well-being. This position offers an exciting opportunity to make a meaningful difference in patients' lives through proactive health management and skilled nursing interventions.

Duties

Conduct thorough patient assessments, including vital signs, physical examinations, and health history reviews
Administer medications via injections, IV infusions, and other routes following strict protocols and HIPAA regulations
Perform wound care, catheterizations, tube feedings, and other specialized procedures with precision and aseptic technique
Coordinate with physicians and interdisciplinary teams to develop and implement individualized care plans
Educate patients and their families on disease management, medication adherence, and lifestyle modifications
Document all patient interactions accurately within EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems such as Epic or Cerner
Provide triage services by assessing urgent needs, managing acute pain cases, and determining appropriate levels of care in diverse environments

Qualifications

Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) with current certification
Proven experience in medical-surgical units, emergency rooms, ICU, or long-term care settings such as nursing homes or assisted living facilities
Strong knowledge of medical terminology, anatomy, physiology, ICD coding (International Classification of Diseases), and medication administration protocols
Experience with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks is highly desirable
Skilled in patient assessment techniques including vital signs measurement and diagnostic evaluation
Ability to work independently in the field while maintaining excellent communication with healthcare teams
Knowledge of infection control practices, HIPAA compliance, and sterile processing procedures
