Full Job Description

Jefferson Healthcare Center is a compassionate and resident-centered long-term care and skilled nursing facility. We are currently seeking an Admissions Coordinator to join our dedicated team. The ideal candidate will bring experience in long-term care admissions and demonstrate a strong ability to multi-task in a fast-paced environment.

Responsibilities

Respond promptly and professionally to all verbal and written inquiries regarding facility placement
Review and process referrals in coordination with the central intake team
Conduct pre-admission interviews (in person, phone, or walk-in) and provide facility tours
Verify insurance benefits and obtain necessary authorizations for admission
Ensure all aspects of the admission process are completed accurately, efficiently, and courteously
Distribute admission documentation to all relevant departments prior to resident arrival
Maintain strong communication and collaboration with other departments to ensure smooth resident transitions
Participate in department meetings and in-service training as required

Skills

Experience in Long-Term Care or Skilled Nursing: 6 months to 1 year
EMR Experience: Proficiency in PCC (PointClickCare) preferred
Customer Service: Exceptional interpersonal and communication skills
Availability: Full-time, with flexibility for occasional extended hours
Strong organizational and multi-tasking abilities
Excellent written and verbal communication skills
Ability to remain composed and professional in a fast-paced environment
Experience working with families and residents during sensitive transitions into care

Pay: $23.00 - $32.00 per hour
