Full Job Description

NOW HIRING: HCBS Case Manager – Arizona DDD

Remote / Work From Home | Full-Time

The Care Agency LLC is looking for an experienced Members HCBS Case Manager who understands Arizona DDD services and knows how to advocate for members and their families.

This is NOT an entry-level position. We need someone who already understands the DDD system and can step into this role with confidence.

REQUIRED

Minimum 1 year of direct experience working with Arizona DDD
Knowledge of HCBS services guidelines and policies.
Experience communicating with DDD Support Coordinators, families, and providers
Excellent written and verbal communication skills
Strong organizational skills with the ability to manage multiple members, deadlines, and priorities
Strong computer skills are an absolute requirement. THIS IS A REMOTE POSITION. Strong computer and office production, software skills are essential for this position.
Proficient with:
Google Workspace / Google Drive
Adobe
DocuSign
Excel (you must be able to create and manager spreadsheets)
Email, HIPPA controls, electronic files, and web-based programs
Ability to learn and navigate multiple software systems quickly
Ability to work independently and problem-solve without constant supervision

V-Track experience is strongly preferred.

WHAT YOU WILL DO

Serve as a primary point of contact for members and families
Assist, support, build relationships and advocate for families to help improve the quality of life of the DDD members we serve
Communicate with DDD Support Coordinators regarding member needs, services, authorizations, and changes
Attend in person meetings with every member in your assigned district.
Monitor authorizations and help prevent non compliant actions.
Write comprehensive progress notes and maintain case records.
Prepare a variety of reports and complete special projects.
Attend staffing meetings, training and other meetings (including statewide travel).
Assist families with questions, concerns, and problems that arise with their services
Communicate and coordinate with caregivers/providers
Monitor service utilization and documentation
Complete forms, correspondence, reports, and other case-related documentation
Follow up on outstanding items and make sure issues are resolved—not simply passed along
Stay organized and keep cases moving forward

THE PERSON WE ARE LOOKING FOR

We need someone who genuinely cares about the families we serve. You should be a strong communicator, advocate, problem-solver, and follow-through person.

You must be comfortable working independently, and be an effective communicator with families, providers, DDD Support Coordinators, and our internal team.

IMPORTANT – COMPUTER SKILLS

This position requires someone who is already highly comfortable using a computer.

We will train you on our agency processes. We will NOT train basic computer skills.

You should already know how to confidently work with Google Drive, shared documents, PDFs, Adobe, DocuSign, email, spreadsheets, electronic filing systems, and multiple browser-based programs throughout the day.

REMOTE POSITION

This is a work-from-home position, but it is a structured full-time job—not a flexible work-at-your-own-pace position.

You must have:

A quiet, professional, distraction-free work environment
Reliable high-speed internet
The ability to remain available and responsive throughout your scheduled shift

SCHEDULE

Choose one consistent shift:

9:00 AM – 6:00 PM

10:00 AM – 7:00 PM

DDD EXPERIENCE IS REQUIRED. V-TRACK EXPERIENCE IS A BIG PLUS.

· 10 paid holidays per year

· Vacation time accrued at 4.00 hours bi-weekly for the first 3 years

· Sick time accrued at 3.70 hours bi-weekly
