Physical Therapist (PT) – Home Health | Flexible Schedule | Competitive Pay

FULL-TIME and/or PART-TIME avail, and per-diem

AREAS INCLUDING: Fort Lee, Englewood, Teaneck, and other areas of Bergen county avail etc. Includes ALF/ILF based patients within community as well!

Ignite Rehab is a growing private practice delivering high-quality Physical Therapy services directly in patients’ homes and community environments. Our clinicians focus on what matters most — helping patients maintain mobility, independence, and overall quality of life where they live.

We’re currently seeking motivated Physical Therapists who want the flexibility, autonomy, and meaningful patient relationships that come with home-based care.

Responsibilities

Provide Physical Therapy services to older adults in home and community settings
Evaluate patient mobility, strength, and functional limitations
Develop and implement individualized treatment plans
Deliver evidence-based therapeutic interventions to improve movement, balance, and independence
Maintain accurate Medicare-compliant documentation
Manage your own schedule and caseload effectively

Ideal Candidate

We’re looking for clinicians who are comfortable working independently in a home health setting and who are organized, motivated, and patient-focused.

Preferred experience:

Medicare documentation and billing
Understanding of the 8-minute rule and CPT coding
Experience managing scheduling and patient caseloads

Requirements

Active Physical Therapy license in the state(s) of practice or eligibility to apply
Basic computer and documentation skills
Current CPR certification

Job Type

Full-time | Part-time | Per Diem opportunities available (W2 and 1099 available!)

Work Setting

Patient homes within the community
Community facilities such as Assisted Living Facilities and/or patient group homes
Home health under Medicare Part B outpatient billing
