Full Job Description

Summary of Position:

This position performs complex planning, research, consultative, technical, and administrative services to support the Area Agency on Aging (AAA) in delivering public education, volunteer coordination, information and referral, and nutrition/transportation program support. The role involves coordinating community resources, providing intake and assessment for service eligibility, developing and delivering educational programs, managing volunteer training, and ensuring compliance with state and federal regulations. The position serves as a liaison between older individuals, their families, and resources to promote independence and quality of life. Work is performed under general supervision with considerable latitude for initiative and independent judgment.

Organizational Relationships

1. : Area Agency on Aging Director.

2. : This is a non-supervisory position.

3. :Maintains regular contact with AAA staff, service providers, local/state/federal agencies, elected officials, regulatory/endorsement agencies, senior organizations, volunteers, clients, their families, and the general public.

nutrition and transportation program specialist:

In conjunction with the Program Specialist, will assist in coordinating with nutrition and transportation contractors to ensure accurate documentation of client eligibility and service authorizations.

data management and reporting:

Ensures accurate, timely, and complete programmatic reporting to comply with state and federal rules, regulations, and contract standards.

Maintains client files, records, and statistical data in accordance with applicable policies, ensuring confidentiality of client information.

Enters data into computerized client tracking systems and statewide databases.

Prepares monthly, annual, or other required reports.

Verifies and submits care coordination vendor billing to finance for payment.

information and referral specialist:

Conducts initial screenings and interviews (via phone or in-person) to assess service needs, determine eligibility, and make appropriate referrals.

Maintains and updates the AAA Resource Guide and website with accurate and timely information in coordination with staff and community partners.

Researches and maintains listings of community resources and providers relevant to older individuals.

public education and outreach:

Plans, coordinates, implements, and evaluates public education needs for the region related to the elderly population.

Maintains agency calendar of outreach and educational events and records data in statewide databases.

Other Important Duties

Performs such other related duties as may be assigned.

Required knowledge, skills, and abilities

Knowledge of: Computer systems, including Windows environment and software such as Microsoft Word and Excel, basic knowledge of aging processes and programs, interviewing techniques, local state and federal regulations relating to human and social service programs, including long-term care, public and private sector resources, general office policies and procedures, and computer operations.

Skill/Ability to: Interpret and communicate to others rules, regulations and guidelines prepared by state and federal agencies relating to aging programs and caregiving; establish and maintain effective working relationships with clients, citizen groups, volunteers and the general public; interview clients skillfully, provide care/client planning skills, evaluate client needs, work independently and in a team setting, and demonstrate proficiency in both oral and written communication. Work as a leader in the community to educate individuals/groups regarding public/private benefits and resources. Bilingual skills preferred.

Acceptable Experience and Training

Bachelor’s degree in social work or a related field; or any equivalent combination of experience and training that provides the required knowledge, skills, and abilities.

Certificates and Licenses required

Appropriate Texas driver’s license or available alternate means of transportation.
