Targeted Case Manager (TCM) — Paintsville Behavioral Health TRP

Location: Hopkinsville, KY

Reports To: Clinical Coordinator

Employment Type: Full-Time

About Us:Paintsville Behavioral Health is a Therapeutic Rehabilitation Program (TRP) dedicated to improving the quality of life for adults experiencing serious mental illness and co-occurring disorders. We provide compassionate, person-centered care focused on recovery, independence, and community integration.

Position Summary:The Targeted Case Manager (TCM) is responsible for providing individualized case management services to TRP participants. This role serves as a vital link between clients, providers, and community supports—ensuring that each participant’s needs are assessed, coordinated, and addressed through a holistic, strengths-based approach. The TCM will also serve as a liaison between the TRP and local Personal Care Homes to promote continuity of care, communication, and advocacy for client needs.

Key Responsibilities:

Conduct comprehensive assessments and develop individualized service plans.

Coordinate and monitor behavioral health, medical, housing, and community services.

Maintain regular contact with clients to assess progress toward recovery goals. Local travel will be required.

Serve as liaison between TRP staff, personal care homes, and outside providers.

Document services and maintain compliance with Medicaid and DBHDID requirements.

Participate in multidisciplinary team meetings and case reviews.

Support client advocacy, empowerment, and community reintegration efforts.

Qualifications:

Bachelor’s degree in a human services-related field (psychology, social work, counseling, etc.) — required.

Current Targeted Case Management (TCM) certification through the Kentucky Department for Behavioral Health, Developmental and Intellectual Disabilities (DBHDID) — required.

Prior experience in behavioral health, social services, or case management preferred.

Strong communication, organization, and documentation skills.

Ability to work collaboratively as part of a multidisciplinary team.
