About the Role
As an LPN at South Mountain Restoration Center, you will provide practical nursing care to individuals with mental or physical illnesses, injuries, behavioral needs, developmental disabilities, or difficulty completing daily living activities.
You will implement established nursing care plans, administer medications and treatments, monitor changes in condition, maintain accurate medical records, and collaborate with registered nurses, physicians, and interdisciplinary treatment teams.
What You’ll Do
Responsibilities may include:
- Providing direct practical nursing care to residents
- Implementing individualized nursing care and treatment plans
- Preparing, administering, and documenting prescribed medications
- Performing ordered treatments and monitoring resident responses
- Observing and reporting changes in physical condition, emotional status, or behavior
- Taking and recording vital signs
- Collecting specimens and assisting with clinical testing
- Maintaining progress notes, medication records, and administrative reports
- Entering resident health information into electronic medical records
- Supporting habilitation and rehabilitation programs
- Assisting residents with daily living, recreational, and leisure activities
- Communicating with registered nurses, physicians, residents, families, and direct-care personnel
- Requesting professional evaluation, treatment, or emergency care when necessary
- Following applicable laws, regulations, facility policies, and nursing standards
- Overseeing unlicensed personnel when assigned charge responsibilities
Licensure-required nursing duties will be performed under the direction of a registered nurse or physician.

The selected LPN should be capable of:
- Providing guidance within the LPN scope of practice

- Troubleshooting resident-care and treatment concerns

Minimum Qualifications
Qualified candidates must have:
- A current Pennsylvania Practical Nurse license or eligibility to sit for the Practical Nurse Licensure Examination
- Proficient computer skills, including Microsoft Office
- The ability to enter resident health information accurately into an electronic medical records database
- Strong practical-nursing and patient-assessment skills
- Effective written and verbal communication abilities
- Strong teamwork, organization, documentation, and safety-awareness skills
- The ability to provide verifiable education, training, licensure, and employment history
- The ability to work every other weekend and 12 designated holidays as scheduled
Required Clinical Competencies

Emergency and Respiratory Care
- CPR, first aid, AED use, and hemorrhage control
- Airway insertion and Ambu bag use
- Oxygen administration
- Peak-flow meter and pulse-oximeter use
- Respiratory-mask use, suctioning, and nebulizer treatments
- Ventilator care
Medication and IV Therapy
- Oral, topical, injectable, inhaled, and suppository medication administration
- Medication-dose calculations
- Intramuscular, intradermal, and subcutaneous injections
- IV therapy, venipuncture, infusion pumps, and heparin-lock care
- Manual flow regulators, tubing management, and secondary medications
- Parenteral therapy
- Acceptance and documentation of licensed practitioner orders within the LPN scope of practice
Assessment and Specimen Collection
- Vital signs and blood-pressure monitoring
- Neurological checks
- EKG and glucometer use
- Audiogram and telebinocular testing
- Tuberculin testing
- Urine dipstick testing
- Collection of blood, urine, stool, wound, and other specimens
Resident Care and Treatment
- Catheter insertion and care
- Perineal and ostomy care
- Dressing changes, wound cultures, drains, and pressure-injury care
- Ear and eye irrigation
- Suture removal
- Sterile technique and wound care
- Isolation and universal or standard precautions
- Restraint procedures
Nutrition, Tubes, and Specialized Care
- Nasogastric and gastrostomy tube insertion, irrigation, and feeding
- Jejunostomy-tube care and enteral nutrition
- Nephrostomy care
- Hoyer-lift techniques
- Mobility and positioning assistance
Supportive Devices and Safety Equipment
- Ace wraps, anti-embolic hose, arm slings, binders, braces, and knee immobilizers
- Traction and K-Pad use
- Evacuation-chair procedures
All duties must be performed within the LPN’s authorized scope and established standards of nursing practice.
