Full Job Description

Lead with purpose and elevate compassionate care as the Director of Palliative Care and Home Health at a thriving regional medical center known for its community feel, large hospital impact and innovation across the continuum of care.

The Director of Palliative Care and Home Health will oversee a dynamic hospice and home-based care service line that blends compassionate end-of-life support with progressive home healthcare solutions. This service area offers the unique advantage of delivering high-acuity care in the comfort of patients’ residences while maintaining strong integration with hospital-based teams. Known for its community feel, large hospital resources, and deeply rooted local support, the program continues to expand access points, enhance home-based technology, and strengthen partnerships that allow patients and families to receive dignified, personalized care wherever they call home.

Reporting to an executive nursing or clinical operations leader, the Director of Palliative Care and Home Health will guide managers, clinical supervisors, and interdisciplinary teams including experienced nurses, therapists, social workers, chaplains, and advanced practice providers. The Director of Palliative Care and Home Health will drive strategic planning, regulatory compliance, quality improvement initiatives, and operational performance while fostering a collaborative, supportive culture. This role is supported by engaged leadership, shared governance structures, and frontline clinicians who value teamwork, professional growth, and compassionate service delivery.
