Full Job Description

Job Purpose:

The Quality Improvement Specialist supports program quality, compliance, and continuous improvement initiatives across LifeStream’s behavioral services. This position is responsible for clinical documentation review, compliance monitoring, quality audits, data analysis, staff support, and performance improvement activities to ensure services are delivered consistently and in alignment with federal, state, contractual, licensing, and CARF accreditation standards.

Key Responsibilities:

Monitor client records and service delivery for compliance with federal, state, contractual, licensing, and CARF accreditation standards.
Review case notes, treatment plans, assessments, medication records, and other clinical documentation for accuracy, completeness, and compliance.
Conduct ongoing monitoring of corrective actions to ensure sustained improvement and compliance.
Prepare reports summarizing audit findings, trends, recommendations, and opportunities for improvement.
Maintain knowledge of current regulatory requirements and CARF accreditation standards.
Work closely with the Vice President of Quality Improvement to strengthen quality systems and internal processes.
Assist with staff training related to documentation standards, compliance requirements, and quality improvement best practices.
Support performance improvement initiatives and monitoring activities across programs.
Participate in quality improvement activities and organizational initiatives as assigned.

Required Education:

Bachelor’s degree in Social Work, Human Services, Psychology, or related field required. Master’s degree preferred.

Required Experience:

3 to 5 years of experience in behavioral health required
Experience reviewing clinical documentation, quality assurance, compliance monitoring, or related quality functions strongly preferred.

Required Skills and Abilities:

Strong analytical skills with the ability to interpret complex data and identify trends.
Excellent communication skills, both written and verbal, with the ability to present findings and recommendations to diverse audiences.
Proficient in using quality improvement tools and methodologies, such as Lean, Six Sigma, or PDSA cycles.
Ability to work collaboratively with multidisciplinary teams to foster a culture of continuous improvement.
Detail-oriented with strong organizational skills to manage multiple projects simultaneously.
Proficiency in Microsoft Office Suite and data visualization software.
