Home and Community Based Services - Community Residential Direct Support Professionals
Board and Lodge with Special Services Mental Health Workers
The Direct Support Professional/Mental Health Worker is directly responsible to the Treatment Director and other supervisors as assigned. Staff are responsible for the implementation of Horizon Homes, Inc. services as identified in the resident’s individual service plans. It is essential that this person maintain a high degree of ethical and professional standards, and has the ability to relate to clients, staff, community members, and related provider entity personnel.
Staff will be crossed-trained in both the Home and Community Based Residential and Board and Lodge with Special Services Programs, and have a working knowledge of both programs as outlined by the State of Minnesota (See References Statutes 245D and 157.17 Below). Orientation, training and oversight will be provided to help the individual understand and demonstrate best practices in their established position with Horizon Homes, Inc. at time of hire and on-going.
This person must be at least 18 years of age with a high school diploma or equivalent. The employee is also expected to successfully complete a minimum of 24 hours of annual training, if they have fewer than 5 years of documented experience, and 12 hours of annual training if 5 or more years of documented training. Vulnerable adults maltreatment reporting and internal review; Maltreatment of minors reporting and internal review; Program abuse and prevention plan; Service recipient rights and staff responsibilities related to ensuring the exercise and protection of those rights; Principles of person-centered service planning and delivery and how they apply to direct support service provided by the staff person; The safe and correct use of manual restraint on an emergency basis according to the requirements in sections [245D.061] and what constitutes the use of restraints, time out, and seclusion, including chemical restraint; Staff responsibilities related to prohibited procedures under section [245D.06] subdivision 5, why such procedures are not effective for reducing or eliminating symptoms or undesired behavior, and why such procedures are not safe; and Other topics as determined necessary in the person’s coordinated service and support plan by the case manager or other areas identified by the license holder.
The individual must have a strong background or interest in the areas of mental illness, chemical dependency, mental health services, and community transition. Organizational skills as well as related work and/or personal experience is beneficial.
This individual must be capable of working independently and will assist the Treatment Director, Nursing Staff, Designated Manager and Designated Coordinator in providing direct care, program, and services.
Tasks and responsibilities are outlined below with the following major areas: Personnel/supervision, direct care services, administrative, and public relations.

Responsibilities include:
I. Personnel/Supervision
A. Complete required orientation provided by Horizon Homes, Inc.
B. Attend all Horizon Homes, Inc. in-service training and education sessions as scheduled monthly.
C. Meet at least monthly with the Designated Manager and Designated Coordinators to review and discuss treatment plans of residents, progress and case notes, treatment plan updates and/or recommended changes in plan; treatment strategies and methods to be implemented; program and services coordination, needs and concerns.
D. Meet with the Designated Manager and Designated Coordinators as appropriate for ongoing supervision, case reviews, and to meet any immediate crisis management and stabilization needs of residents.
E. Assist as appropriate with orientation of new staff.
II. Direct Care Services
A. Maintain contact with each resident as agreed upon in the program plans.
B. Notify Designated Manager and Designated Coordinators as appropriate to address any concerns or difficulties with the resident, particularly those involving the residents’ health and well-being.
C. Assist each resident in determining his/her goals and objectives whenever possible.
D. Maintain resident file, progress notes, and any other required records.
E. Assist the resident in ensuring that his/her rights and confidentiality are respected and not violated.
F. Monitor the resident in the administration of medication.
G. Assess and help educate the resident in emergency procedures: fires, tornado, and severe weather conditions.
H. Assist the resident in the following areas as determined in resident individual service plans including but not limited to the following:
1. Menu-planning, grocery shopping, and food preparation.
2. Ensure that the resident meets acceptable standards of grooming and hygiene and dresses appropriately for the weather conditions.
3. Ensure the resident makes and follows through with medical appointments and the maintenance of good health.
4. Assist the resident in structuring free time; provide leisure and recreation opportunities on an individual basis and in a group setting.
5. Assist the resident in money management and finances.
6. Ensure the resident maintains his/her home and assist with housekeeping as appropriate.
7. Assist the resident in applying for government services, such as financial assistance, food stamps, subsidized housing, etc.
8. Provide support to resident in continuing recovery and relapse prevention, sobriety.
9. Provide on-call assistance for psychiatric and medical emergencies
10. Attend resident referral, intake, team, and discharge meetings as needed.
11. Maintain records of time spent with each resident.
12. Participate in on-going staff training including direct supervisory hours provided by the Designated Manager and Designated Coordinators, group in-service trainings monthly, and other training/educational opportunities.
III. Administrative
A. Assist with the development and implementation of the home community-based services program and services available to residents.
B. Participate in the admissions and reviews of the residents.
C. Assist with the Program Evaluation, Consumer Satisfaction Survey Process, and completion of these forms.
D. Maintain current knowledge and compliance with county, state, and federal rules, regulations, and mandates.
E. Meet as scheduled with the Treatment Director, Designated Manager, and Designated
Coordinators.
IV. Public Relations
A. Develop and maintain alliances with the community including:
1. Public agencies
2. Private practitioners and individuals
3. Local churches, business, industries
V. Other
A. Maintain awareness of local, state, and national affairs that concern the professional field.
B. Maintain a positive community image.
C. Maintain an advocacy position for resident services and rights.
D. Respect resident confidentiality at all times.
E. Provide on-call crisis intervention services as deemed appropriate.
F. Perform any other duties assigned by the Designated Manager, Treatment Director, Designated Coordinators, Nursing Staff, Assistant Executive Director, and Executive Director.

Pay: $16.00 - $19.00 per hour
