Therapy at Home (TAH) Position Overview

Therapy at Home (TAH) is seeking a thoughtful, adaptable Physical Therapist to join our interdisciplinary team serving older adults in assisted living, memory care, and facility-based environments.

This role is ideal for clinicians who value patient-centered care, professional collaboration, and clinical growth, and who understand the unique responsibility and privilege of working with geriatric populations, including individuals with dementia and complex medical needs.

Core Clinical Responsibilities

Provide skilled occupational therapy services to geriatric patients across varying diagnoses and functional levels
Adapt treatment plans based on daily patient presentation, including cognition, stamina, mood, and medical status
Support functional independence, safety, participation, and quality of life
Deliver care that may be restorative, wellness-focused, or maintenance-based depending on patient needs
Understand the cycle of care and how each visit fits into the patient’s broader plan

Emotional Intelligence & Patient-Centered Care

Recognize and respond appropriately to changing patient emotional states
Communicate effectively with:
Patients
Families and POAs
Facility staff
Interdisciplinary therapy teams
Determine what information is essential to share with families and facilities, recognizing that care needs are not “one-size-fits-all”
Preserve patient dignity, autonomy, and trust in all interactions

Collaboration & Communication

Work collaboratively with OTs, SLPs, nursing staff, aides, intake, and leadership
Participate in coordinated care planning and interdisciplinary communication
Accept feedback and align with organizational care standards
Communicate proactively within facilities to support continuity of care and appropriate referrals

Documentation & Professional Accountability

Complete timely, ethical, and defensible documentation
Understand unit-based billing and productivity expectations
Adapt to documentation platforms, including WebPT and EPIC
Balance efficiency with clinical quality, safety, and compliance

Support, Supervision & Mentorship

Qualifications

Bachelor’s or Master’s degree in Physiotherapy or an equivalent qualification recognized in the healthcare industry.
Valid licensure or registration as a licensed Physical Therapist in the relevant jurisdiction.
Proven experience working in inpatient hospital settings, outpatient clinics, or home health environments.
Strong knowledge of physiology, anatomy, medical terminology, and therapeutic techniques such as trigger point therapy.
Experience with EMR systems for documentation and patient management.
Familiarity with care planning for diverse populations including pediatrics, geriatrics, orthopedics, and neurological conditions.
