What You'll Be Doing
- Provide comprehensive primary care throughout senior living communities.
- Perform routine provider visits and comprehensive patient assessments.
- Diagnose and manage acute and chronic medical conditions.
- Develop individualized treatment plans.
- Coordinate care following hospitalizations and rehabilitation stays.
- Collaborate with physicians, nurses, therapists, social workers, and caregivers.
- Educate patients and families on disease management and preventative care.
- Participate in specialty programs and quality improvement initiatives.
- Help reduce avoidable hospitalizations and emergency department visits.
- Become a trusted clinical resource for residents, families, and community staff.

What We're Looking For
We're searching for a compassionate, relationship-driven Nurse Practitioner who enjoys practicing independently while collaborating with an experienced interdisciplinary team.
Required Qualifications
- Active Minnesota Nurse Practitioner license in good standing.
- Strong assessment, diagnostic, and clinical decision-making skills.
- Ability to practice independently while collaborating with physicians.
- Passion for caring for older adults and medically complex patients.
- Reliable transportation and willingness to travel throughout the Rochester region.
Preferred Qualifications
- Experience managing chronic medical conditions.
- Background in senior living, post-acute care, or community-based medicine.
- Advanced wound care experience.
- Wound Care Certification (WCC) preferred.
