Full Job Description

Job Overview
We are seeking a dedicated and compassionate Physical Therapist, PRN to join our healthcare team. This flexible position offers the opportunity to provide high-quality patient care across various settings, including inpatient, outpatient, home health, and community care. The ideal candidate will possess strong clinical skills, a thorough understanding of physiology and anatomy, and experience working with diverse patient populations such as pediatrics, geriatrics, orthopedics, and patients recovering from acute care or hospital stays. This role emphasizes personalized care plans and discharge planning to support patient recovery and independence.

Duties

Assess patients’ physical conditions through comprehensive evaluations, including mobility, strength, and functional status.
Develop and implement individualized care plans tailored to each patient’s needs, goals, and medical history.
Provide therapeutic interventions and other modalities to improve mobility, reduce pain, and restore function.
Coordinate discharge planning by collaborating with healthcare teams to ensure seamless transition of care from hospital to home or community settings.
Document patient progress accurately using EMR systems and communicate effectively with physicians, caregivers, and other healthcare professionals.
Educate patients and their families on exercises, injury prevention, and home care strategies to promote recovery outside clinical settings.
Stay current with medical terminology updates, advances in physiotherapy techniques, and best practices in patient care.

Requirements

Valid licensure as a Physical Therapist with current state registration or certification.
Proven experience in hospital, home health, or outpatient clinic settings; experience in inpatient or acute care environments is highly desirable.
Strong knowledge of physiology, anatomy, and relevant medical terminology.
Familiarity with various treatment modalities including trigger point therapy and orthopedic rehabilitation techniques.
Ability to develop comprehensive care plans and participate actively in discharge planning processes.
Proficiency in using EMR systems for documentation and communication purposes.
Excellent communication skills with the ability to educate patients effectively about their treatment plans.
Prior experience working in outpatient clinics, hospital settings, or home & community care environments is advantageous. Pay: $75.00 - $85.00 per hour
