Full Job Description

Senior MDS Coordinator RN

We are seeking a talented and dedicated Senior MDS Coordinator RN to join our team!

As a Senior MDS Coordinator RN you will play a vital role in ensuring the smooth operation of the department through traveling to each of our skilled nursing facilities and providing support. Your primary responsibility will be to serve as the liaison between the healthcare team, patients, and families, while also coordinating the Resident Assessment Instrument (RAI) process. We're looking for a dynamic and organized professional who is passionate about delivering high-quality care and has excellent communication skills.

Responsibilities:

Coordinate and facilitate the RAI process, ensuring that all necessary data is collected and documentation is complete and accurate
Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient
Conduct initial assessments, assessments updates, and comprehensive assessments in a timely and efficient manner
Gather and document patient data from various sources, including medical records, nurse's notes, and family reports
Develop and maintain relationships with patients, families, and healthcare providers to ensure seamless coordination of care
Participate in quality improvement initiatives and quality assurance programs to ensure compliance with regulatory requirements and industry standards

Requirements:

Current RN licensure in the state of IL
Minimum of 5 years of experience in a skilled nursing setting
Strong knowledge of the Resident Assessment Instrument (RAI) process and Federal Requirements
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced environment and prioritize multiple tasks simultaneously.
Ability to travel to our skilled nursing facilities throughout the Chicagoland area & suburbs.
