Full Job Description

Job Locations: Absecon, Northfield, Somers Point and Woodbine

Job Overview:

Oversee and manage residential group home facilities in the community. Assist and support in the day-to-day operations of Residential Program and oversight of Cornell, Cedar bridge, Highland Group Homes, California Supervised Apartments and Woodbine Supervised Apartments. Responsibilities include helping ensure staff members are providing quality care to service recipients, providing direct supervision to managers, and assisting with staff training and development. Complete and compile accurate, detailed daily documentation. Actively participate in various decision-making processes including interviewing, staff hiring, termination resulting from investigation, budget development, fiscal management, and facility maintenance. Strong written and verbal communication skills. Detail oriented, focused, dependable and a strong commitment to teamwork. Works as a team participant to ensure the property meets the quality maintenance standards set by U.S. Department of Housing and Urban Development, the Department of Human Services Office of Licensing, the Division of Developmental Disabilities.

Essential Duties and Responsibilities:

· Will oversee Residential Programs including all Residential Responsibilities up to and including:

· Residential Month End Submissions for each location pertaining to: Client Account Records, Expense Reports, Vehicle Mileage Report, Petty Cash, Produce Ledger, Residential Monthly Grocery Request, Shop Rite Expenditures Report, SNAP/Food Stamp Report, Total Care Card

· Residential Quarterly Program Audits

· Residential Policies & Procedures; implementation and review

· Residential Unannounced Site Visits.

· Residential Inventory (collected from managers)

· Residential Asset Management

· Residential Emergency Management Plans

· Residential Performance Evaluations

· Residential monthly review of program budgets

· Residential Financial Oversight and Audit

· Coordinates the development and implementation of all standards as defined by The Department of Human Services, Office of Licensing.

· Coordinates and ensures the implementation of all Division Circulars as defined by the Division of Developmental Disabilities.

· A requirement that the employee cooperate with the licensee and Department of Human Services staff in any inspection, inquiry or investigation.

· Licensing Annual Site Inspections.

· Oversee Programmatic Policies & Procedures, SP & CCP.

· Oversee managers, monitoring annual renewal dates for Individual Service Plans.

· Coordinate adaptive equipment, diet and other required training with Agency nurse

· Emergency on-call for residential programs

· Cover shifts in managers’ absence

· Community Quest Inc. responsibilities including:

· CQI Annual Residential Program Evaluation

· CQI Outcomes Measures Report

· CQI Satisfaction Survey Community and Staff CQ

· CQI Business Function Analysis Action Plan for each fiscal year CQ

· CQI Calendar

· CQI Facilitate Strategic Plan CQ

· CQI Concerns for Improvement Process

· CQI Grievance Procedure that coincides with HR

· Must have the ability to effectively and accurately communicate with service recipients and their families. Including providing direct assistance required by service recipients and their families in a professional and timely manner.

· Perform on call duty.

· Payroll for all residential programs as needed.

· Maintenance Requests: Receiving, Assessing, Strategizing, Gathering Bids & Requesting Approval from Finance and oversight of entire major maintenance process.

· ISP Coordinator for assigned Group Homes and Supervised Apartments; -develops and implements ISPs.

· Ensure service recipients health needs are being met; schedule, follow up and support individuals on appointments as needed.

· Ensure timely incident reporting and follow-up reporting according to Department of Human Services (DHS) and Division of Developmental Disabilities (DDD) guidelines and circulars.

· Identifies and assists in the provision of pertinent documentation as per Agency and Division requirements including staff development, hiring, training, and evaluating residential aide staff and managers.

· Audit Individual Supports Case Notes for all individuals served. Review all case notes and match up to DDD attendance report at month end.

· Audit menus, medical consultation records and medications administration records ensuring compliance with written orders and procedures as set by Agency and DDD.

· Group Home Daily Log Inspection (weekly and monthly)

· Cooperates with Department of Human Services (DHS) staff in any inspection or investigation.

· Complete quarterly quality audit reports

· Submits monthly reports to Finance Department and complies with all Fiscal Policies.

· Heavy lifting, transporting of individuals and moving of equipment, etc. may be required.

· Documents on Individual Service Recording system.

· Familiarize with SNAP Program and monitor food budget for service recipients.

· Familiarize and Oversee Stephen Komninos Law and communication requirements

· Health and Safety Committee member; ensures compliance with Agency Policies and Procedures and Health and Safety Policies and Procedures

· Responsible for the accuracy of Medication Administration Record Sheets (MARS) and inventory

· Audit Medication Administration Record Sheets (MARS)

· Coordinate and plan team meetings for individuals served

· Build, implement, maintain and follow SOP’s

· Volunteer in agency fundraising events and sit on agency committees as needed. This includes H&S Committee Chair, involving the coordination and oversight of:

· Ensuring Agency Policies and Procedures and H&S Policies and Procedures are being utilized.

· H&S 6 Month UIR Report Sign Off & Approval

· H&S Inspection Schedule

· H&S Physical Inspections of all residential locations and administrative office(s) as needed

· H&S Service Recipient file inspection

· H&S Vehicle Emergency Plans

· H&S Management Plan Annual Audit

· H&S Emergency Evacuation and Fire Drill Analysis

· Valid NJ driver’s license and access to transportation to work

A requirement that the employee agree to have their name checked against the Central Registry of Offenders Against Individuals with Developmental Disabilities

· Must be certified or certifiable under N.J.A.C 10:44A. Standards for Community Residences staff training requirements which includes successfully completing the New Jersey Pre-Service Trainings: Cardio-pulmonary resuscitation (CPR), American Red Cross Standard First Aid, Overview of Developmental Disabilities, Medication, and Preventing Abuse and Neglect

A requirement that the employee cooperate with the licensee and Department of Human Services staff in any inspection, inquiry, or investigation
Employee shall submit to drug testing prior to employment
Employee shall submit to drug testing conducted randomly and for cause
Employee shall complete an application to have their name checked against the Child Abuse Registry Information (CARI)
Employee must successfully complete and demonstrate proficiency in all areas of required training.

Job Requirements:

High School Diploma (or equivalent), plus experience with developmentally disabled persons and s supervisory experience in the field of human services. Strong leadership and organizational skills.
