NATURE OF POSITION: The Behavioral Health Case Manager II is an integral member of Central Oklahoma Family Medical Center (COFMC), Inc.

Knowledge of current social and economic problems, principles of family
economics and budgeting, human relation, community, state and national health and welfare resources, social assistance requirements and psychological

Completion, or eligible to enroll, of the curriculum requirements for a Behavioral
Health Case Manager II (BHCM II) certification for the State of Oklahoma
-
Completion of the curriculum requirement for a Bachelor’s degree from an
accredited university in a mental health related field, or a current license as a registered nurse in the State of Oklahoma
-
Ability to demonstrate knowledge base as indicated by the COFMC Competency
Assessment for Behavioral Healthcare Practitioners
-
Knowledge of theoretical principles of Psychosocial Rehabilitation and knowledge of appropriate client recreational and group rehab activities
-
Knowledge of emergency procedures, clinical policies and procedures, and infection control procedures
-
Previous supervisory experience preferred
-
Customer service experience required Ability to work and communicate with a multidisciplinary healthcare team and the public

Expanded computer skills; required

Must be able to sit, stand, walk, bend, and/or lift for extended periods of time

Ability to demonstrate knowledge base as indicated by the COFMC Competency

terminology.

Knowledge of emergency procedures, clinical policies and procedures, and infection control procedures.

Knowledge of theoretical principles of Psychosocial Rehabilitation and knowledge of appropriate client recreational and group rehab activities.

Attend all meetings, trainings, and/or other events as deemed mandatory by the
PSR Supervisor, Director of Behavioral Health, and/or by COFMC Admin.
-
Document all other pertinent client information on a “BH Case Note” either in the
paper chart or in the client EHR in accordance to the acting practice of the BH department in a timely and efficient manner.

Document services rendered in a weekly progress note through the use of an
electronic health record (EHR) in accordance to the acting practice of the
Behavioral Health department in a timely and efficient manner.

Keep accurate van logs and document services rendered in accordance with
COFMC and the State of Oklahoma policies and procedures and submit monthly for review.
-
Keep accurate weekly billing sheets and submit for review in a timely manner.
-
Always maintain BHCM certification and remain in good standing with the State of Oklahoma.

Participate in treatment team review meetings as they pertain to client services.
-
Provide psychosocial education to SPMI clients regarding adult daily living skills
(ADL’s; personal hygiene, housekeeping, budgeting) and basic living skills (self advocacy, health/safety, and goal setting) through the format and/or use of
group discussion, group work, handouts, video(s) and outings.

Provide transportation services to client(s) as assigned by the PSR Supervisor.
