Full Job Description

Position Summary

We are seeking a detail-oriented and compassionate MDS Coordinator (RN) to join our long-term care team. The MDS Coordinator is responsible for coordinating the Resident Assessment Instrument (RAI) process, completing accurate and timely MDS assessments, developing individualized care plans, and ensuring compliance with federal and state regulations. The ideal candidate is organized, knowledgeable in Medicare and Medicaid reimbursement, and committed to delivering quality resident care.

Essential Duties and Responsibilities

Complete accurate and timely MDS assessments in accordance with CMS guidelines.
Coordinate the interdisciplinary assessment and care planning process.
Ensure all OBRA, PPS, and other required assessments are completed within regulatory timeframes.
Monitor and maintain MDS scheduling to ensure compliance.
Develop, review, and update comprehensive resident care plans.
Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members.
Participate in Medicare meetings and reimbursement reviews.
Review clinical documentation to ensure accuracy and support coding.
Monitor quality measures and implement strategies to improve outcomes.
Assist with regulatory surveys and audits.
Educate nursing staff on MDS documentation requirements and regulatory changes.
Maintain confidentiality and comply with HIPAA regulations.
Perform other duties as assigned by the Director of Nursing.

Qualifications

Current, unrestricted RN.
MDS experience in a skilled nursing or long-term care setting preferred.
Knowledge of CMS regulations, RAI process, PDPM, Medicare, and Medicaid reimbursement.
Strong assessment, documentation, and organizational skills.
Excellent communication and teamwork abilities.
Proficient with electronic medical records (EMR) systems.
