Full Job Description

Job Summary
Join our compassionate healthcare team as a Hospice RN Case Manager, where you will play a vital role in delivering personalized end-of-life care that emphasizes dignity, comfort, and quality of life. In this dynamic position, you will coordinate patient care plans, provide expert clinical assessments, and support families through challenging times. Your expertise in acute and post-acute care settings will ensure seamless transitions and comprehensive symptom management, making a meaningful difference in patients’ final journeys.

Duties

Conduct thorough patient assessments to determine eligibility and develop individualized care plans aligned with hospice standards.
Manage and coordinate comprehensive symptom control, including pain management and comfort measures, in collaboration with interdisciplinary teams.
Provide education and emotional support to patients and their families, guiding them through complex medical decisions and end-of-life planning.
Oversee discharge planning, ensuring smooth transitions from hospital or inpatient settings to home or outpatient hospice services.
Document all patient interactions accurately in medical records while adhering to HIPAA regulations to protect patient privacy.
Complete all IDG Comprehensive Assessments, RN Comprehensive assessments, HOPE visits, as assigned- daily.
Participate in utilization management and diagnostic evaluation processes to ensure appropriate resource use and quality outcomes.

Requirements

Valid RN license with current registration; experience in critical care settings such as ICU, emergency medicine, or trauma centers highly preferred.
Demonstrated expertise in hospital experience including medical-surgical units, post-acute care environments, outpatient clinics, or inpatient facilities.
Strong knowledge of physiology, medical terminology, and patient assessment techniques essential for complex case management.
Experience working with diverse populations including geriatrics, pediatrics (PICU), behavioral health patients, and those with infectious disease concerns.
Proficiency in discharge planning, medical records documentation, intake procedures, vital signs monitoring, and utilization management.
Ability to navigate HIPAA compliance while managing sensitive patient information effectively.
Excellent communication skills with a focus on patient service and compassionate care delivery.
Additional experience in managed care settings or outpatient clinic environments is a plus.
