Full Job Description

Overview
We are seeking compassionate and dedicated Direct Support Professionals to provide high-quality care and assistance to individuals with developmental disabilities, aging needs, or health challenges. This role involves supporting residents in their daily activities, ensuring safety, promoting independence, and enhancing overall well-being within a supportive environment. The ideal candidate will have a strong commitment to person-centered care and possess relevant experience in home and community care settings.

Responsibilities

Provide hands-on support to individuals with daily living skills, including personal care, meal preparation, medication administration, and transportation.
Support individuals in participating in community activities, recreational outings, employment, and skill-building programs.
Implement individualized service plans and behavioral support strategies as directed.
Promote dignity, respect, and choice in all interactions.
Maintain a clean, safe, and supportive environment in compliance with agency standards and regulatory requirements.
Complete required documentation accurately and in a timely manner (e.g., progress notes, incident reports, medication logs).
Assist with medical appointments, therapy sessions, and other personal needs as necessary.
Respond appropriately to emergencies and follow all safety procedures and protocols.
Communicate effectively with individuals, families, team members, and supervisors.
Attend all required trainings, meetings, and professional development sessions.

Skills

Prior experience in group homes, assisted living, nursing homes, or home health care preferred
Knowledge of HIPAA regulations and patient privacy standards
Certification in CPR and First Aid required; additional training in Alzheimer's or dementia care is a plus
Strong observational skills for patient monitoring and safety assessment
Ability to assist with ADLs effectively while demonstrating patience and compassion
Experience working with individuals with developmental disabilities or autism spectrum disorders highly desirable
Capable of heavy lifting and physical assistance using Hoyer lifts or other mobility aids
Excellent communication skills for working with residents, families, and healthcare teams
Ability to follow detailed care plans accurately and document observations thoroughly
