Full Job Description

Job Summary

Join our dynamic healthcare team as a PRN (as-needed) Physical Therapist, where you will play a vital role in delivering exceptional patient care across diverse settings. In this flexible position, you will assess, develop, and implement personalized treatment plans to promote optimal recovery and mobility. Your expertise will support patients of all ages, including geriatric, pediatric, neurological, orthopedic, and outpatient populations. This role offers the opportunity to collaborate with multidisciplinary teams, utilize your clinical skills, and make a meaningful difference in patients' lives—all while maintaining a flexible schedule that fits your lifestyle.

Duties

Evaluate and treat, as necessary, hospice patients that reside in homes, ALs, and SNFs
Conduct comprehensive patient evaluations to determine functional limitations and develop tailored treatment plans aligned with clinical standards.
Deliver evidence-based physiotherapy interventions such as manual therapy, trigger point therapy, therapeutic exercises, and mobility training to facilitate recovery.
Manage discharge planning processes by coordinating with healthcare professionals and ensuring seamless transitions for patients returning home or to community care settings.
Document patient progress accurately in electronic health records (EHR) systems following clinical documentation standards and medical terminology guidelines.
Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare providers to optimize patient outcomes through interdisciplinary care planning.
Assist in outpatient clinic or home health environments by providing patient rehabilitation services that address orthopedic, neurological, pediatric, or geriatric needs.
Educate patients and their families on treatment procedures, post-treatment care plans, and exercises to promote ongoing recovery and independence.

Qualifications

Valid Ohio state licensure or certification as a licensed Physical Therapist practicing in a physical therapy role.
Proven experience working with diverse patient populations including geriatrics, pediatrics, orthopedics, neurology, or outpatient rehabilitation centers.
Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards necessary for accurate record keeping.
Experience with inpatient hospital settings, outpatient clinics, home health environments or outpatient rehabilitation centers is highly preferred.
Familiarity with electronic health records (EHR) or electronic medical records (EMR) systems for documentation management.
Demonstrated ability to develop effective treatment plans and modify them based on patient progress and response.
Excellent client communication skills with the ability to motivate and educate patients throughout their recovery process.
