Case Manager – Mercy House

Mercy House, part of IPHNY, is looking for a compassionate, motivated, and organized Case Manager to join our team. This is more than just a job—it's an opportunity to empower individuals experiencing homelessness by helping them regain stability, achieve their goals, and build a path toward permanent housing and independence.

What You'll Do

As a Case Manager, you'll serve as an advocate, coach, and resource for the individuals we support by:

Developing individualized service plans based on each guest's strengths, needs, and goals.
Meeting regularly with guests to monitor progress and celebrate successes.
Assisting guests in obtaining stable housing, employment, healthcare, benefits, identification, income, and community resources.
Coordinating services with community agencies, healthcare providers, mental health professionals, substance use providers, and other partners.
Maintaining accurate and timely documentation in electronic record systems while ensuring compliance with agency and funding requirements.
Conducting assessments, completing required paperwork, and maintaining confidential records.
Assisting with crisis intervention, conflict resolution, and problem-solving while maintaining a trauma-informed approach.
Helping guests develop life skills that promote long-term independence and self-sufficiency.
Participating in case conferences, team meetings, and interdisciplinary collaboration.
Assisting with program reporting, audits, and quality improvement initiatives.
Building positive relationships with guests while maintaining professional boundaries.
Supporting a safe, welcoming, and respectful shelter environment.

Experience with any of the following is a plus:

Homeless services
Housing navigation
Case management
Mental health
Substance use services
Crisis intervention
Trauma-informed care
Motivational Interviewing
Harm reduction practices
